鼻唇沟皮瓣在美学修复上唇组织缺损中的应用

摘　　要：目的探讨鼻唇沟皮瓣在美学修复上唇组织缺损中的应用原则、效果和意义。方法用鼻唇沟皮瓣对28例不同原因致上唇组织缺损患者进行修复，皮瓣沿鼻唇沟自然曲线走行方向进行美学设计，在无张力情况下旋转皮瓣修复创面。其中男性和女性分别为12例和16例，年龄范围是18～76岁，平均年龄（46±18）岁。组织缺损最小者为1．0cm×1．3cm，最大者为2．5cm×3．2cm，将其分为左侧为Groupl（GI）共13例和右侧为Group2（G2）共15例，术后6个月填写调查问卷（满意度Likert5点量表）。结果28例患者，皮瓣均I期愈合，术后上唇形态满意度良好，局部结构平整，瘢痕隐蔽性良好，无器官移位异常，功能良好，随访6个月，美学评价均较高。而相对比而言，瘢痕隐蔽性方面的评价较低。结论根据鼻唇沟区自然曲线特征，设计不同形式的鼻唇沟皮瓣修复上唇皮肤缺损，手术简单、可靠，围手术期短，可同时达到较好的功能恢复和美学视觉效果。目的探讨鼻唇沟皮瓣在美学修复上唇组织缺损中的应用原则、效果和意义。方法用鼻唇沟皮瓣对28例不同原因致上唇组织缺损患者进行修复，皮瓣沿鼻唇沟自然曲线走行方向进行美学设计，在无张力情况下旋转皮瓣修复创面。其中男性和女性分别为12例和16例，年龄范围是18～76岁，平均年龄（46±18）岁。组织缺损最小者为1．0cm×1．3cm，最大者为2．5cm×3．2cm，将其分为左侧为Groupl（GI）共13例和右侧为Group2（G2）共15例，术后6个月填写调查问卷（满意度Likert5点量表）。结果28例患者，皮瓣均I期愈合，术后上唇形态满意度良好，局部结构平整，瘢痕隐蔽性良好，无器官移位异常，功能良好，随访6个月，美学评价均较高。而相对比而言，瘢痕隐蔽性方面的评价较低。结论根据鼻唇沟区自然曲线特征，设计不同形式的鼻唇沟�Objective To evaluate the designing principles,postoperative results and significance of repairing the tissue defects of upper lip in the aspect of aesthetic by nasolabial flap. Methods By using nasolabial flap doing the repairment for upper lip tissue defects in 28 cases which were caused by different reasons, all flaps were designed according to the aesthetic natural lines of nasalabial, and twist the flap to repair the defects without any tense. There were 12 male and 16 female , aged from 18 to 76 years,with a mean age of （46 ~ 18） years. The size of tissue defect ranged from I. 0cm x 1.3cm to 2.5cm x 3.2em. Divided them into two groups ,the Left side is group 1 with 13 cases and the right side is group 2 with 15cases. And 6 months later every patient finish one piece of satisfaction papers. Results The flaps are all survived by primary healing postoperatively. After 6 months following up in 28 cases,the patients all were satisfied with the upper lip shapes, hidden scar, non organs removing and good function. And they all satisfied with the aesthetic results. The mean rating for scar visibility on a 5 - point scale was lower than that of the other aspects. Conclusion It is very＂ useful of using the nasolabial flap for repairing the upper lip tissue defects not only in functiong reconstruction,but also in the aspect of satisfied aesthetic re- suits, short time costing, easy to have the skill and so on. The upper lip tissue defects should be repaired individually according to the features of natural lines.
