H型高血压与缺血性脑卒中的相关性研究

摘　　要：目的研究H性高血压与缺血性脑卒中的相关性。方法连续入选我院缺血性脑卒中患者264例,记录患者一般资料、既往史、个人史、影像学检查、实验室检查(血糖、血脂、同型半胱氨酸)及治疗情况,6个月后电话随访进行MRS评分。结果 264例患者,6个月随访率95%,入院后同型半胱氨酸平均水平为(19.04±11.20)μmol/L。高同型半胱氨酸组与非高同型半胱氨酸组患者在年龄、性别及高血压的发生率方面比较,差异有统计学意义(P<0.05)。Logistic多因素回归分析显示,高同型半胱氨酸血症界值为10μmol/L,H型高血压与脑卒中6个月MRS评分不相关,而高同型半胱氨酸血症界值为15μmol/L,H型高血压与缺血性脑卒中6个月MRS明显相关(P<0.05)。结论高血压与高同型半胱氨酸血症是缺血性脑卒中的危险因素,且有协同作用,与患者6个月MRS评分显著相关。Objective To study the correlation between the H hypertension and ischemic stroke. Methods A total of 264 cases of ischemic cerebral stroke patients in our hospital, record the general data of patients, medical history, personal history, imaging, laboratory examination（blood glucose, blood lipid, plasma homocysteine） and treatment, after 6 months of telephone follow-up MRS score. Results Of the 264 patients, 6 months follow-up after hospital admission rate of 95%, the average level of homocysteine is（19.04 ＋ 11.20） mol/L. High homocysteine group and non high homocysteine groups in rate comparison age, gender and hypertension, the difference was statistically significant（P〈0.05）. Logistic multivariate regression analysis showed, hyperhomocysteinemia boundary value is 10 not related to mol/L, H type of hypertension and stroke 6 months MRS score, and hyperhomocysteinemia boundary value is 15 mol/L, H was significantly correlated to hypertension and ischemic stroke 6 months MRS（P〈0.05）. Conclusion Hypertension and high homocysteine is a risk factor for ischemic stroke, and have a synergistic effect, significantly correlated with 6 months for patients with MRS score.
