摘  要:目的探讨振动反应成像(vibration response imaging,VRI)技术在支气管哮喘和慢性阻塞性肺疾病(chronic obstructive pulmonary diseases,COPD)诊断中的应用价值。方法回顾性分析2012年6月至2015年1月在海南医学院附属医院呼吸内科接受VRI检查的30例支气管哮喘患者(哮喘组)、COPD患者(COPD组)和健康志愿者(对照组)的VRI图像,比较3组的最大振动能量图(maximum energy frame,MEF)、动态图像、振动能量曲线、声波信号包络(envelope of acoustic signal,EVP)曲线、图像总评分、曲线振动能量值和右侧肺部定量数值(quantitative lung data,QLD)。结果与对照组比较,哮喘组MEF、动态图像、振动能量曲线、EVP曲线及图像总评分升高,差异有统计学意义(P<0.05),曲线振动能量值和右侧QLD值升高,差异无统计学意义(P>0.05);COPD组MEF、动态图像、振动能量曲线、EVP曲线及图像总评分升高,差异有统计学意义(P<0.05),曲线振动能量值降低,右侧QLD值升高,差异无统计学意义(P>0.05)。与COPD组比较,哮喘组动态图像、振动能量曲线、曲线振动能量值和图像总评分升高,差异有统计学意义(P<0.05),EVP曲线和右侧QLD值升高,差异无统计学意义(P>0.05)。结论动态图像、振动能量曲线、曲线振动能量值和图像总评分可协助诊断与鉴别诊断支气管哮喘和COPD患者。Objective To explore the value of vibration response imaging( VRI) in the diagnosis of bronchial asthma and chronic obstructive pulmonary diseases( COPD). Methods A retrospective study was performed on 10 bronchial asthma patients,10 COPD patients and 10 healthy volunteers who undergoing VRI in our hospital from June 2012 to January 2015. Their VRI including maximum energy frame( MEF),dynamic image,respiratory curve,envelope of acoustic signal( EVP),total scores of dynamic imaging,the highest value of vibrational energy and the total quantitative lung data( QLD) value of right lung among bronchial asthma,COPD and healthy controls were analyzed. Results Compared with the healthy controls,MEF,dynamic image,respiratory curve,EVP curve and total scores of dynamic imaging were significantly higher in bronchial asthma patients( P〈0. 05),but the highest value of vibrational energy and the total QLD value of right lung were higher,but without significant difference( P〉0. 05). MEF,dynamic image,respiratory curve,EVP curve and total scores of dynamic imaging were significantly higher in COPD patients( P〈0. 05),but the highest value of vibrational energy was increased and the total QLD value of right lung was decreased though both having no statistical significance( P〉0. 05). The bronchial asthma group had obviously higher dynamic image,respiratory curve,the highest value of vibrational energy and total scores of dynamic imaging than the COPD group( P〈0. 05). But no significant difference was seen in the EVP curve and the total QLD of right lung between the 2 groups( P〉0. 05). Conclusion Dynamic imaging,respiratory curve,the highest value of vibrational energy and total scores of dynamic imaging from VRI are helpful in the diagnosis and differential diagnosis between bronchial asthma and COPD.
