抗凝治疗下肢深静脉血栓后髂股静脉影像学表现

摘　　要：目的观察下肢深静脉血栓患者经抗凝治疗6~12个月后髂股静脉影像学表现。方法选取87例下肢深静脉血栓形成患者,对亚急性期血栓患者进行传统开放取栓手术,急性期血栓患者行经皮腘静脉置管溶栓术,术后服用华法林抗凝治疗6~12个月并定期随访。行经皮股静脉途径造影、观察髂股静脉影像学表现。结果造影显示5例患者股静脉完全闭塞;72例患者存在不同程度的髂股静脉狭窄,狭窄程度30%~90%,其中69例盆腔大量侧支开放;6例患者髂股静脉完全通畅;4例患者髂总静脉闭塞。结论下肢深静脉血栓患者经抗凝治疗后仍存在髂股静脉不同程度的狭窄,影像学表现可为治疗血栓后综合征提供帮助。Objective To observe the images of iliac femoral vein in patients with deep venous thrombosis after anticoagulant therapy for 6—12months.Methods Totally 87 patients with deep venous thrombosis were treated with open surgery（subacute stage）or persistent transcatheter thrombolysis through popliteal vein（acute stage）.Then the patients were given warfarin and followed up for 6—12months.And then the images of iliac femoral vein were observed through percutaneous femoral vein angiography.Results The femoral vein occlusion appeared in 5cases,72 cases appeared iliac femoral vein stenosis in different degrees（30%—90%）and pelvic collateral circulation opened in 69 cases,the iliac femoral vein received complete recanalization in 6cases,and the iliac femoral vein occlusion appeared in 4cases.Conclusion After anticoagulant therapy,the patients with deep venous thrombosis still appeares iliac femoral vein stenosis in different degrees.The images of deep vein after anticoagulant therapy is helpful for treatment of post-thrombotic syndrome.
