摘 要:目的:回顾性分析2型糖尿病患者糖化血红蛋白(HbA1c)、同型半胱氨酸(Hcy)、凝血指标的关系。方法:152例2型糖尿病患者以糖化血红蛋白6.5%为临界,分为血糖控制良好组(HbA1c〈6.5%)及血糖控制不良组(HbA1c〉6.5%),同时,选取30例健康体检者为正常对照组,比较各组间同型半胱氨酸(Hcy)及凝血指标(PT, APTT ,TT, FIB-C)的差异,并分析各指标与糖化血红蛋白水平的相关性。结果:与对照组比较,糖尿病控制良好组 Hcy显著增高(P〈0.05),凝血指标变化不具有统计学意义;糖尿病控制不良组较其他两组,Hcy显著增高(P〈0.01), APTT显著下降(P〈0.05),FIB-C显著上升(P〈0.05);糖尿病患者同型半胱氨酸(Hcy)水平、APTT缩短与糖化血红蛋白(HbA1c)呈正相关。结论:血浆Hcy水平及凝血功能异常可能是导致糖尿病并发症的危险因素。Objective:To observe relational changes of plasma glycated hemoglobin (HbA1c),homocysteine(Hcy) and coagulation in patients with type 2 diabetes.Methods:152 cases of patients with type 2 diabetes were divided into good glycemic control group(HbA1c〈6.5% ) and poor glycemic control group (HbA1c〉6.5%), and 30 cases of normal controls were collected. Plasma glycated hemoglobin (HbA1c), homocysteine(Hcy) ,coagulation parameters (PT, APTT, TT, FIB-C) were measured and compared among the three groups, in addition, the correlations between Hcy ,PT, APTT, TT, FIB-C and HbA1c were analyzed. Results: Hcy of good glycemic control group was higher than normal group(P〈 0.05), while the changes of PT, APTT, TT, FIB - C were not obvious. Compared with good glycemic control group and normal control group, poor glycemic control group showed significantly higher Hcy (P〈 0.01) ,shorter APTT(P〈 0.05) and elevated FIB-C(P〈 0.05). The result displayed the positive correlations between Hcy ,APTT shortening and HbA1c of the patients with type 2 diabetes.Conclusions:Plasma Hcy levels and abnormal coagulation could be the risk factor of diabetes complications.
