三级评价机制在临床输血管理中的应用

摘　　要：目的：探讨三级评价机制在临床输血管理中的应用效果，为医疗机构临床输血管理提供参考。方法：回顾性地分析本院2011年-2014年临床用血情况，并对临床输血重点指标和临床医疗业务指标进行统计学分析。结果：2012年和2013年，在出院人次分别增长1.34%、18.82%；手术人次增长7.05%、8.55%的情况下，受血者人数分别下降5.68%、19.67%；年用血量分别下降13.8%、15.31%。2014年在受血者人数和手术人次增长0.4%、13.52%的情况下，血浆用量处于负增长（-5.74%），而血小板和冷沉淀用量大幅增长，分别为73.58%、94.97%。临床输血重点指标受血者人数、输血量与临床医疗业务指标出院人次、手术人次比较，差异具有统计学意义（P〈0.05）。结论：通过建立输血三级评价机制，强化了临床输血管理，提高临床用血水平，保证临床用血科学、合理、安全、有效，值得推广。Objective：To investigate the effect of three evaluation mechanisms applied in the clinical blood transfusion management in order to offer reference for clinical transfusion management of medical institutions.Methods： Retrospective analysis of our hospital blood for clinical cases in 2011 and 2014, and focused on clinical indicators and clinical transfusion business metrics for statistical analysis.Results：2012 and 2013 years, with the 1.34%, 18.82% increasing of dischared patients and 7.05%, 8.55% increasing of surgery patients, the number of recipients fell 5.68%, 19.67%; the amount of blood transfusion decreased 13.80%, 15.31%. 2014 respectively,with the 0.40% and 13.52% increasing of the number of recipients and surgical patients,the amount of plasma in negative growth （-5.74%）, while the amount of platelets and cryoprecipitate substantial growth, respectively, 73.58%, 94.97%. Key indicators of clinical transfusion recipients, blood transfusion and clinical business metrics dischared patients ,patients surgery , the difference was statistically significant（P 〈0.05）.Conclusions：Through the establishment of blood transfusion three evaluation mechanism, we have strengthen the clinical blood transfusion management, improved the levels of clinical use of blood, ensure the scientific ,reasonable, safe, effective and worthy of promotion.
