血清维生素D水平与小儿下呼吸道感染及其严重程度的关系

摘　　要：目的：探讨血清25-羟维生素D3[25-（OH）D3]水平与小儿急性下呼吸道感染（ALRI）及其严重程度的关系及意义。方法：选取2013年12月至2014年10月在我院儿科住院确诊的115例下呼吸道感染患儿，分为毛细支气管炎51例、轻症肺炎组36例与重症肺炎组28例，采用化学发光法测定其血清中25-（OH）D3水平，选择同期在我院儿童保健科体检的60例健康小儿作为对照组。结果：（1）各组血清25-羟维生素D3水平分别为：重症肺炎组（15.89±6.65）ng /L、轻症肺炎组（23.95±6.45）ng /L、毛细支气管炎组（26.37±6.50）ng /L、对照组（36.42±9.36）ng /L，ALRI三组的血清25-（OH） D3水平与对照组比较差异均有统计学意义（P＜0.05），ALRI三组各组间比较差异均有统计学意义（P＜0.05）。（2）重症肺炎组维生素D不足发生率为96.4%，显著高于对照组的26.7%（P＜0.01）。结论：小儿急性下呼吸道感染血清维生素D水平低于健康儿童；重症肺炎患儿血清维生素D水平显著低于健康儿童。因此，血清维生素D水平与小儿下呼吸道感染及其严重程度有密切的关系，及时补充维生素D对预防和治疗小儿急性下呼吸道感染有重要的意义。Objective：To investigate the correlations between the serum 25-hydrovitamin D3 [25-（ OH） D3]level and the lower respiratory infection and its Severity in children.Methods： Detect the serum 25-（ OH） D3 of one hundred and fifteen hospitalized children with lower respiratory infection by using chemiluminescence during December 2013 to October 2014.They were divided into a capillary bronchitis group,a mild pneumonia group and a severe pneumonia group,while sixty healthy children were enrolled as a control group.Results：（1） The average levels of 25-（ OH） D3 of the severe pneumonia group,the mild pneumonia group,the capilary bronchitis group and the control group were （15.89±6.65）ng /L,（23.95±6.45）ng /L,（26.37±6.50）ng /L,（36.42±9.36） ng /L respectively,the difference between the groups was remarkable （P〈0.05） ; （ 2）The Vitamin D levels in children with severe pneumonia group was significantly lower than that of the control group （P〈0.01）.Conclusions：The levels of Vitamin D in Children with acute lower respiratory infection were lower than that of healthy children; This phenomenon is more obvious in children with severe pneumonia.therefore, Serum vitamin D levels has a close relationship with lower respiratory infection and its serious degree in children, Vitamin D supplements is very important for the prevention and treatment of children with acute lower respiratory infection.
