三妙散与天麻钩藤饮联合西药治疗皮肤型结节性多动脉炎1例报告  

San Miao Powder and Tian Ma Gou Teng Yin Joint Bozhi Glycopeptide Injection and Pidotimod Treatment of CPAN One Case

摘　　要：报告1例三妙散+天麻钩藤饮+薄芝糖肽+乐松+匹多莫德+泮托拉唑钠等中西医结合治疗皮肤型结节性多动脉炎,连续治疗17d,红斑结节基本消退,无新发。女,58岁,双下肢结节红斑3个月。双腿伸侧、屈侧皮肤均可见,最大直径6cm,皮温高,触痛明显,未用药。血压190/100mm Hg。RF:60.3IU/m L↑,ASO:448.3IU/m L↑,HBs Ag>250.00IU/m L,HBV-DNA2.180E+07IU/m L↑;ANA:阳性;结核菌素试验:强阳性,T-spot.TB:阴性。符合1990年美国风湿病学学会(ACR)结节性多动脉炎(PAN)诊断标准。薄芝糖肽静点、乐松,1片/次,3次/d;匹多莫德1瓶/d;泮托拉唑钠肠溶胶囊1片/d。口干、口苦,腰酸,舌红苔黄,脉濡滑,辨为湿热阻滞、肝阳上亢,湿热痹阻痹症,清热除湿,平肝潜阳,三妙散合天麻钩藤饮(苍术15g,黄柏、牛膝、天麻、钩藤、石决明、黄芩、杜仲、独活各20g,羌活15g,土茯苓20g,金银花15g,白花蛇舌草、仙鹤草各30g,赤芍15g,红曲1袋),1剂/d。连续治疗17d,下肢结节红斑基本消退,实验室检查:RF:50.4IU/m L↑,ASO:397IU/m L↑,病情好转出院。继续抗病毒、保肝、调节免疫治疗。[Objective]Report 1 case Sanmiao powder and Tianma Gou Teng Yin Join Bozhi glycopeptide Injection and Pidotimod that Chinese and Western medicine combined treatment of cutaneous polyarteritis nodosa,continuous treatment of 17D,erythema nodules basic subsided,no new disease.Female,58 years old,double lower limb erythema erythema 3 months.Both the legs and the flexion side of the skin were visible,the maximum diameter of 6cm,high temperature,tenderness,no medication.Blood pressure 190/IOOmmHg, RF：60.3IU/ mL↑ ,ASO：448.3IU/mL ↑ ,HBsAg〉 250.00IU/ml,HBV-DNA 2.180E＋07IU/mL ↑ ; ANA ： positive,Str0ng positive tuberculin test：T-spot.TB：negative.With the 1990 American Institute of Rheumatology（ACR）polyarteritis nodosa （PAN）diagnostic criteria.Bozhitangtai static point,Lesong,1 tablets each time,3 times/d;1 bottles of Pi do Maude /d;Pantoprazole Sodium Enteric-coated Capsules 1/d.Dry mouth, mouth pain,backache,red tongue,pulse moisten slip,differentiation of damp heat blockage,hyperactivity of liver Yang,syndrome of damp heat blockage,Desiccant heat,calming liver and suppressing yang,Sanmiao powder combining with Tianma Gouteng Decoction（Atractylodes 15g,Cortex Phellodendri,Radix Achyranthis Bidentatae,gastrodia tuber, hooked uncaria,eoncha haliotidis,baikal skullcap root,eucommia bark,Radix Angelicae Pubescentis,20g,notopterygium root 15g,Poria cocos 20g,honeysuckle 15g,spreading hedyotis herb,hairyvein agrimony 30g,Radix Paeoniae Rubra 15g,Monascus 1 bag）,one dose daily.Continuous treatment of 17,lower extremity nodule erythema subsided,laboratory examination：RF：50.4IU/mL arrows,ASO：397IU/mL arrows,the condition improved discharge. Continue to anti-virus,liver,immune therapy.
