摘 要:目的探讨润目灵颗粒治疗水液缺乏性干眼的作用机制。方法用1%硫酸阿托品滴眼液滴眼制备兔水液缺乏性干眼模型,给予润目灵颗粒治疗14 d。对全部实验兔行Schirmer I试验,测定泪膜破裂时间,观察角膜染色,光、电镜观察其泪腺形态学的变化,免疫组化法对泪腺组织中凋亡相关基因蛋白进行检测。结果与模型组比较,Schirmer I试验检测显示,润目灵组泪液量明显增加(P<0.01),泪膜破裂时间明显延长(P<0.05,P<0.01),角膜染色分值明显减小(P<0.01),泪腺导管及腺泡上皮细胞中凋亡相关因子(factor-related apoptosis,Fas)及其配体(Fas ligand,Fas L)阳性表达的细胞数明显减少(P<0.05),泪腺细胞的形态学损伤明显减轻。结论润目灵颗粒可能通过抑制水液缺乏性干眼兔泪腺组织的细胞凋亡及腺体萎缩,达增加泪液分泌、维持泪膜稳定的作用。Objective To explore the possible therapeutic mechanism of Runmuling granules for aqueous tear deficiency dry eye rabbits. Methods Rabbit model of aqueous tear deficiency dry eye was induced with 1% atropine sulfate eye drops. Runmuling treatment group was treated with Runmuling granules for 14 days after the establishment of dry eye model. Schirmer I test was performed for the detection of tear film breakup time (BUT) and lacrimal secretion. Corneal fiuorescein staining scores were evaluated under light and electron microscope for the observation of lacrimal gland morphology, and immunohistochemieal method was used for the detection of protein levels of apoptosis-related factors of Fas, FasL, Bcl-2 and Bax in lacrimal gland of all rabbits. Results Compared with the model group, the secretion of tear was increased(P 〈 0.01), tear film BUT was prolonged(P 〈 0.05, P 〈 0.01 ), corneal fluoreseein staining scores were reduced(P 〈 0.01 ), the number of lacrimal epithelium cells with Fas and FasL positive expression was decreased (P 〈 0.05), and the morphological damage of lacrimal cells was obviously relieved in Runrnuling group. Conclusion Runmuling could significantly improve the tear flow and tear film stability of lacrimal gland by inhibiting the apoptosis and gland atrophy of lacrimal gland tissue of dry eye rabbit model.
