不同年龄段小儿支原体肺炎的临床特征分析

摘　　要：目的探讨不同年龄段小儿支原体肺炎的临床特征。方法选取成都市高新区芳草社区卫生服务中心2012年10月—2014年10月收治的支原体肺炎患儿87例,根据年龄段划分为两个组,0~3岁为婴幼儿组,4-14岁为年长儿组。对比分析两组患儿临床特征。结果两组患儿发热、白细胞计数增多、肺外并发症发生例数比较,差异无统计学意义（P〉0.05）,婴幼儿组患儿干咳例数少于年长组,喘鸣或肺部湿啰音例数多于年长组,差异有统计学意义（P〈0.05）。两组患儿浸润部位比较,差异有统计学意义（P〈0.001）。结论婴幼儿支原体肺炎感染呼吸道症状明显,湿啰音、喘鸣音等肺部体征明显,X线主要表现为小叶实质浸润性病变,肺外并发症较少;年长患儿一般高热持续时间较长,以刺激性干咳为主,主要损害部位为肺段实质,X线下可见自肺门向肺野外呈放射状分布的大片絮状阴影,其中右肺最为常见。Objective To investigate the clinical characteristics of children with mycoplasma pneumonia（MPP）in different age groups. Methods In Fangcao Community Health Service Center of Gaoxin District from October 2012 to October 2014,a total of 87 children with MPP were selected,they were divided into infant group（0 - 3 years old）and older children group（4 - 14 years old）by age groups. The clinical characteristics of two groups were compared and analyzed. Results The cases of fever,WBC count increased,outside the lung illness complication between the two groups were compared,the differ-ences were not statistically significant（P ﹥ 0. 05）,dry cough children in infant group were less than older children group, wheeze and moist rale were more than older children group,the differences were statistically significant（ P ﹤ 0. 001）. The mainly invasion sites between the two groups were compared,the differences were not statistically significant（P ﹥ 0. 05）. Con-clusion Respiratory symptom are signficant in infant with MPP,moist rale,wheeze are signficant,lobular parenchymal infil-trative lesions are the major performance of X - ray,have less outside the lung illness complication;duration of high fever is lon-ger in older children,irritable dry cough is major,X - ray is visible from the lung to lung field radiated large flocculent shadow of the distribution,most common in right lung.
