阿托伐他汀钙对冠心病患者疗效及血清中细胞因子TNF-α、CRP、IL-4和IL-10的影响

摘　　要：目的：观察阿托伐他汀钙治疗冠心病患者的疗效，并分析其对血清细胞因子肿瘤坏死因子（TNF-α）、C反应蛋白（CRP）、白细胞介素-4（IL-4）和白细胞介素-10（IL-10）的影响。方法选择本院于2013年10月至2014年4月收治的冠心病患者78例作为研究对象，心功能分级NYHA Ⅰ～Ⅱ级，随机分为两组，对照组采取氨氯地平治疗，观察组给予氨氯地平联合阿托伐他汀钙片治疗，均治疗8周，比较两组疗效差异，并行细胞因子TNF-α、CRP、IL-4和IL-10测定，比较其水平差异。结果观察组总有效率92.3%（36/39），显著高于对照组（χ2=5.571，P＜0.05）。观察组治疗后TC、TG、LDL-C水平分别为（3.01±0.27）mmol/L、（1.87±0.21）mmol/L、（2.02±0.15） mmol/L，均低于对照组（P＜0.05）；观察组HDL-C水平高于对照组（P＜0.05）。治疗后观察组TNF-α、CRP、IL-4水平分别为（54.7±21.1）pg/ml、（3.1±1.1）mg/L、（22.1±7.8）ng/L，均低于对照组（P＜0.05）；观察组IL-10水平高于对照组（P＜0.05）。结论阿托伐他汀钙治疗冠心病患者疗效较佳，调脂效果较好，还可改善细胞因子异常改变，具有重要临床价值。ObjectiveTo observe the effect of atorvastatin calcium in the treatment of patients with coronary artery disease, and to analyze its effect on serum cytokines tumor necrosis factor alpha （TNF-α）, C reactive protein （CRP）, interleukin-4 （IL-4） and interleukin-10 （IL-10）.Methods Selected 78 patients with coronary heart disease treated in the hospital in October 2013-April 2014 as the objects of study with heart function grade NYHAⅠ-Ⅱ grade, they were randomly divided into two groups, the control group took amlodipine treatment, the observation group was given amlodipine combined with Atorvastatin Calcium Tablets treatment, all were treated for 8 weeks, then to compare curative effect between the two groups, and TNF-α, CRP, IL-4 and IL-10 were measured, then to compare the level. Results The total efficiency in the observation group were 92.3% （36 cases） and was significantly higher than the control group （χ2=5.571,P〈0.05）. The observation group’s TC, TG, LDL-C levels after treatment were （3.01±0.27） mmol/L, （1.87±0.21） mmol/L, （2.02±0.15） mmol/L and were lower than the control group （P〈0.05）; but HDL-C level was higher than the control group （P〈0.05）. The observation group’ TNF-α, CRP, IL-4 levels were （54.7±21.1） pg/ml, （3.1±1.1） mg/L, （22.1±7.8） ng/L and were lower than the control group （P〈0.05）; but IL-10 level was higher than the control group （P〈0.05）.Conclusion Atorvastatin calcium in the treatment of patients with coronary artery disease has better effect and better lipid regulating effect, also it can improve the abnormal changes of cytokines, so it has important clinical value.
