经阴道超声提示盆腔积液深度与腹腔内出血量的相关性研究

摘　　要：目的探讨经阴道超声提示盆腔积液深度与腹腔内出血量的相关性。方法选择我院因异位妊娠行手术治疗的168例患者,以术前阴道超声子宫直肠陷凹游离液区最大深度的测定值(即阴道超声测定值)与术中盆腔积血量作相关回归分析,试图建立计算盆腔积液量的回归模型。结果盆腔积血≤400mL时,阴道超声测定值与盆腔积血量,符合直线回归方程:Y(盆腔积血量估计值)=9.829 X(阴道超声测定值)+6.67,两者呈显著正相关(r=0.945,P<0.01);盆腔积血>400mL时,上述两参数间不存在线性关系,直线回归分析结果显示无明显相关性(r=0.382,P>0.05)。结论对于盆腔积液≤400mL时,可由术前阴道超声测定值估计盆腔积血量,以进一步指导临床工作;当盆腔积液>400mL时不宜以阴道超声子宫直肠陷凹单个平面的测定值来估计腹腔积液量,应以多个积液平面的多参数做多元回归分析。Objective To investigate the relationship between the pelvic free liquid indicated by transvaginal ultrasound and the intra-abdominal hemorrhage.Methods One hundred and sixty-eight ectopic pregnancy patients in our hospital were selected for examination by ultrasound before operation.We analyzed the maximum depth of the uterus rectal pouch free fluid（by transvaginal ultrasound）and the intra-abdominal hemorrhage to establish a regression model for calculating the amount of pelvic fluid.Results When pelvic hemorrhage≤400mL,the vaginal ultrasound measured values was considered to be linear with the pelvic hemorrhage,which were fitted the regression equation：Y=9.829 X＋6.67.That′s to say,the quantity of free liquid in pelvis possess a significant positive correlation with the actual quantity of blood in pelvis.And it has statistical significance（r=0.945,P〈0.01）.When pelvic hemorrhage 〉400 mL,the two parameters were considered to be have no relations.And according to the analysis about linear regression,the two parts has no significant relations（r=0.382,P〈0.05）.Conclusion When the pelvic fluid≤400mL,the pelvic free liquid indicated by ultrasound could indicate the pelvic hemorrhage.It will provide an important evidence to guide our clinical work.When pelvic fluid 〉400 mL,we can′t estimate the whole pelvic free liquid with a single plane in uterus rectum.Instead,the multi-parameter of multiple effusion plane should be used for regression analysis.
