摘 要:目的探讨血清细胞因子在亚临床甲减(SCH)合并妊娠期糖尿病(GDM)孕妇中的表达及临床意义。方法选取2012年8月—2014年10月在唐山市妇幼保健院妇产科检查的SCH合并GDM孕妇48例为观察组,另选同期健康体检孕妇50例为对照组。采用贝克曼DXI800化学发光分析仪检测促甲状腺素(TSH);ELISA法检测血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α);运用贝克曼AU5800全自动生化分析仪检测超敏C反应蛋白(hs-CRP),对结果进行分析比较。结果亚临床甲减合并妊娠期糖尿病孕妇组TNF-α、IL-6、hs-CRP的水平高于对照组,差异有统计学意义(t=4.694-9.165,P〈0.05);TNF-α、IL-6、hs-CRP与TSH水平呈正相关(r=0.295-0.386,P〈0.05)。结论细胞因子TNF-α、IL-6、hs-CRP可能通过多种机制参与孕妇SCH合并GDM的发病,并对孕妇SCH合并GDM具有一定的预测、评估价值。Objective To explore the expression and clinical significances of cytokines in gestational diabetes womencomplicated with subclinical hypothyroidism. Methods Forty-eight gestational diabetes women complicated with subclinicalhypothyroidism in Tangshan Municipal Maternal Hospital were selected as the observation group between August 2012 andOctober 2014.ELISA was employed to test concentrations of TNF-α and IL-6.ECLI was used to detect concentrations of TSH and hs-CRP with AU5800 Synchron Clinical System. Results The serum concentrations of TNF-α,IL-6 and hs-CRP of thepatients in the observation group was higher than those in the control group(t=4.694-9.165,P〈0.05).The concentrations ofserum TNF-α,IL-6 and hs-CRP were positively correlated with that of serum TSH(r =0.295-0.386,P〈0.05). Conclusion TNF-α,IL-6 and hs-CRP might involved in the development and progression of gestational diabetes patients complicatedwith subclinical hypothyroidism by variety of mechanisms.The concentrations of TNF- α,IL-6 and hs-CRP possess certainvalues in prediction of gestational diabetes patients complicated with subclinical hypothyroidism.
