运用追踪方法学评价中山市人民医院外科Ⅰ类切口手术预防性应用抗菌药物的规范性

摘　　要：目的:运用追踪方法学评价中山市人民医院(以下简称"我院")外科Ⅰ类切口手术预防性应用抗菌药物的规范性。方法:利用医院信息系统随机抽取若我院600份不同病种Ⅰ类切口手术的归档病历,追踪手术医师是否经过相关培训;预防用药指征、给药时间、品种选择等是否恰当;临床药师在手术预防性应用抗菌药物管理中的技术支撑作用;手术与预防性应用抗菌药物物的质量控制指标及绩效考核制度及指标。结果:我院Ⅰ类切口手术围术期抗菌药物预防性使用率为8.67%(52/600),在适应证、药物选择、用法与用量、给药时间等各方面均较合理。结论:通过追踪方法学评价发现,我院手术医师经过抗菌药物合理应用培训,加之临床药师对围术期预防性应用抗菌药物的干预,Ⅰ类切口手术抗菌药物的使用基本合理。OBJECTIVE：To apply tracer methodology in the evaluation of the use of prophylactic antibiotics in surgical patients undergoing typeⅠincision operation in Zhongshan Municipal People＆#39;s Hospital （ hereinafter referred to as“our hospital”） .METHODS：Using hospital information system, a total of 600 filed medical records of patients （ with different diseases） undergoing typeⅠincision operation in our hospital were randomly collected for retrospective analysis regarding whether or not the surgical doctors have received related training, whether the indiction, administration time and drug selection were rational, the technical support role of clinical pharmacists in the control of the perioperative use of prophylactic antibiotics; the quality control indexes of operation and prophylactic use of antibiotics, the performance appraisal systems and indicators.RESULTS：Among the 600 patietns undergoing typeⅠincision operation in our hospital, the rate of perioperative prophylactic use of antibiotics was 8.67%（52/600） .The antibitoic use was raitonal in indications, drug selection, dosage and administration, administraiton time etc.＆amp;nbsp;CONCLUSIONS：The evaluation by tracer methodology revealed that due to the training among surgical doctors about rational antibotic use plus clinial pharmacists＆#39;intervetion on perioperative prophylactic use of antibiotics, the antibiotic use in patients undergoing type Ⅰincision operation was rational on the whole.
