精子DNA碎片率、精子形态率与精子顶体酶之间的关系及其对妊娠结局影响的研究

摘　　要：目的探讨精子DNA碎片率与精子形态率、精子顶体酶三者之间的关系及其对妊娠结局的影响。方法采用精子染色质扩散实验(SCD)对100例接受试管助孕的男方进行精子DNA碎片率(DFI)检测,按照WHO标准进行精液常规分析,将精子DFI、精子形态率、精子顶体酶和精子活力以及前向运动精子活率进行Spearman相关性检验,将精子DFI对妊娠结局的影响进行卡方检验分析。结果精子DNA碎片率与精子形态率具有相关性,而其他两者无相关性。前向运动精子活率与精子DNA碎片率差异有统计学意义(P<0.05),精子活力与精子DNA碎片率差异有统计学意义(P<0.05)。精子DNA碎片率对妊娠结局没有影响。结论精子DNA碎片率影响前向运动精子活率和精子活力,而对优质胚胎率、妊娠结局没有影响。Objective To explore the relationships between sperm DNA fragmentation rate and sperm morphology rate,sperm acrosin and the impacts on pregnancy outcome. Methods Sperm chromatin diffusion test（ SCD） was used to detect sperm DNA fragmentation rate of 100 male patients receiving assisted pregnancy; routine semen analysis was conducted according to WHO standard,the correlations between sperm DNA fragmentation rate and sperm morphology rate,sperm acrosin,sperm motility,forward movement sperm viability were analyzed by Spearman correlation test; the effect of sperm DNA fragmentation rate on pregnancy outcome was analyzed by Chi- square test.Results There was a correlation between sperm DNA fragmentation rate and sperm morphology rate,but sperm DNA fragmentation rate was not correlated with sperm motility and forward movement sperm viability. There was statistically significant difference in forward movement sperm viability and sperm DNA fragmentation rate（ P〈0. 05）,but there was no statistically significant difference in sperm motility and sperm DNA fragmentation rate（ P〈0. 05）. Sperm DNA fragmentation rate had no impact on pregnancy outcome. Conclusion Sperm DNA fragmentation rate has impact on forward movement sperm viability and sperm motility,but it has no impact on high quality embryo rate and pregnancy outcome.
