高血压颈动脉粥样硬化证候要素诊断量表的研制

摘　　要：目的:运用文献研究和专家咨询问卷调查的方法初步确定高血压颈动脉粥样硬化的常见证素及相关对应条目。方法:结合文献研究,在对证候诊断和证候评价进行界定的基础上,制订专家咨询问卷,通过2轮专家问卷调查数据,采用均数、变异系数、累积百分比评价四诊信息条目的诊断重要性,筛选常见证素及相关条目池。结果:本研究按照累积百分比>60%及变异系数<0.35为标准,筛选出3个病位证素,5个病性证素并相关的对应条目。结论:高血压颈动脉粥样硬化的常见证候要素为肝、肾、脾、痰证、血瘀、阴虚、火热、阳亢,符合该病本虚标实的证候规律与临床所见。同时,本研究将有助于实现高血压颈动脉粥样硬化中医辨证研究暨中医药疗效评价研究的规范化。Objectives： Trough the methods of document researches and expert consultation questionnaire to initially determine the common syndromes and related items of hypertensive CAS. Methods： Based on the document researches and the definition of syndrome diagnosis and syndrome evaluation, expert consultation questionnaire, which was carried out 2 times,was formulated to get the survey data. By taking advantage of the diagnostic importance of mean, coefficient of variation and cumulative percentage evaluation of four diagnostic methods information were used to select the common syn- dromes and related item pool. Results： In the study, acco^ing to the standard of cumulative percentage greater than 60% and coefficient of variation less than 0.35 ,three syndrome factors of disease location and five syndrome factors of disease nature and related corresponding items were selected. Conclusion： The common syndrome factors of hypertensive CAS includes liver, kidney, spleen, phlegm, blood stasis, deficiency of Yin, heat and hyperactivity of Yang,which conforms to the syndrome rules of deficiency in origin and enrichment in superficiality and the clinical symptom. At the same time, this study will contribute to the standardization of the study of TCM syndrome and the research of the evaluation of the curative effect of TCM of hypertensive CAS.
