鼻内镜手术治疗非侵袭性真菌性鼻窦炎38例临床分析

摘　　要：目的：探讨鼻内镜手术治疗非侵袭性真菌性鼻窦炎的临床疗效。方法选取2010年2月～2013年1月我院收治的38例非侵袭性真菌性鼻窦炎患者作为研究对象，分析治疗前后患者的VAS评分及Lund-Kennedy评分，观察手术疗效及术后并发症情况。结果治疗前VAS为（5.76±1.13）分，Lund-Kennedy为（7.82±2.30）分；治疗后1个月VAS为（4.24±1.06）分，Lund-Kennedy为（4.36±2.03）分；随访结束时VAS为（2.21±0.87）分，Lund-Kennedy为（1.49±1.38）分。治疗后1个月及随访结束时两种量化评分与治疗前对比，P＜0.05，差异具有统计学意义。随访结束时两种量化评分低于治疗后1个月， P＜0.05，差异具有统计学意义。38例患者术后治愈36例（94.7%），好转1例（2.6%），无效1例（2.6%），治愈率为94.7%，有效率为97.4%。38例患者均未出现脑脊液鼻漏、鼻泪管阻塞等并发症，腔内未见脓状分泌物及真菌团块，鼻塞、脓涕、涕中带血、头痛等症状均消失。结论采用鼻内镜手术治疗非侵袭性真菌性鼻窦炎疗效显著，可有效避免术后复发。Objective To explore the clinical effect of endoscopic sinus surgery for noninvasive fungal sinusitis. Methods Selected 38 cases of non-invasive fungal sinusitis from February 2010 to January 2013 in our hospital as the research object, analysis before and after the treatment of nasal obstruction, purulent nasal discharge, nasal discharge with blood, headache and other symptoms disappeared. Results In the treatment of vas （5.76±1.13）, Lund Kennedy for （7.82±2.30）, treatment after 1 month, the VAS for （4.24±1.06） and Lund Kennedy as （4.36±2.03）, at the end of follow-up VAS （2.21±0.87） and Lund Kennedy as （1.49±1.38）. After 1 months of treatment and the end of follow-up, two kinds of quantitative score compared with the treatment before, P〈0.05, the difference was statistically significant. At the end of the follow-up, the two quantitative scores were lower than the 1 months after treatment, P〈0.05, had difference statistically significance. 38 patients were cured in 36 cases （94.7%）, 1 cases （2.6%）, 1 cases （2.6%）, the cure rate was 94.7%, the effective rate was 97.4%. 38 cases of cerebrospinal rhinorrhea, nasolacrimal duct obstruction and other complications were not seen in patients, the cavity no purulent secretions and fungal mass, nasal obstruction, purulent nasal discharge, nasal discharge with blood, had a headache wait for a symptom disappeared. Conclusion Endoscopic sinus surgery is effective in the treatment of non-invasive fungal sinusitis, and can effectively prevent postoperative recurrence.
