小儿支气管肺炎的相关炎症因子水平变化情况

摘　　要：目的探讨小儿支气管肺炎的相关炎症因子水平变化情况。方法将90例小儿支气管肺炎患儿分为重症支气管肺炎组(n=36例),轻型支气管肺炎组(n=54例),选取同期在我院进行健康体检儿童40例作为对照组进行对照研究。结果治疗前重症支气管肺炎组、轻型支气管肺炎组及健康对照组受检者的WBC、CPR及PCT水平逐渐下降,两两比较均P<0.001;治疗后两组患儿的WBC、CRP、IL-6及PCT水平均显著低于治疗前,P<0.001;重症支气管炎患儿治疗前及治疗后的WBC、CRP、IL-6及PCT水平均显著高于轻型支气管肺炎组患儿同期水平,P<0.001;重症支气管肺炎的CRP与IL-6(r=0.619),CRP与PCT(r=0.442),IL-6与PCT(0.390)均存在明显的正相关性,P<0.05;轻型支气管肺炎的CRP与IL-6(r=0.582),CRP与PCT(r=0.420),IL-6与PCT(r=0.324)均存在明显的正相关性,P<0.05。肺炎患儿的WBC与CRP、IL-6及PCT无明显相关性,P>0.05。结论小儿支气管肺炎CRP、IL-6及PCT水平测定可作为炎症强弱及肺炎严重程度的预警指标,对于指导治疗及预后判断有一定的帮助。Objective To explore the changes of the related inflammatory factors level in Children with bronchial pneumonia. Methods 90 cases of Children with bronchial pneumonia were divided into the severe bronchial pneumonia group （n= 36patients）, Light bronchial pneumonia group （n=54patients）. select 40 cases of Health examination children in our hospital in the same period as the control group. Results The WBC、CRP、PCT in the severe bronchial pneumonia group, Light bronchial pneumonia group and the control group were graduallyfall. Pairwise comparisons were P〈0. 001. the CRP, IL-6 and PCT levels in the severe bronchial pneu-monia group were significantly higher than that in the Light bronchial pneumonia group, P〈0. 001. the CRP and IL-6 （r=0. 619）, CRP and PCT （r=0. 442）, IL-6 and PCT（0. 390） presence significant positive correlation in the severe bronchial pneumonia group. P〈0. 05. the CRP and IL-6 （r=0. 582）, CRP and PCT （r=0. 420）, IL-6 and PCT （r=0. 324） presence significant positive corre-lation in the the light bronchial pneumonia group. P〈0. 05. the wbc compared with CRP, IL-6 and PCT didn＇t presence significant positive correlation in children with bronchial pneumonia. Conclusion The CRP, IL-6 and PCT levels measured in Children with bronchial pneumonia could as the intensity and extent of the pneumonia severity . and early warning indicators of inflammation,there is some help for guiding treatment and prognosis.
