正常小肠及其系膜血管MSCT强化特征的定量研究

摘　　要：目的定量观测多层螺旋CT(MSCT)正常小肠强化参数,为小肠疾病的诊断提供参考指标。方法选择43名成人健康体检者作为研究对象,采用MSCT行腹部平扫和动脉期、门静脉期及平衡期动态增强扫描,分别在平扫和增强扫描图像上测定肠系膜上动脉、肠系膜上静脉、十二指肠、空肠及回肠的CT值。结果平扫各段小肠CT值之间的比较无统计学意义(P>0.05),动态增强扫描各段小肠均以门静脉期强化最显著(P<0.01),各时期各段小肠CT值之间的差异也均有统计学意义(P<0.01),强化程度由高到低依次是十二指肠、空肠和回肠。各时期十二指肠、空肠、回肠CT值与肠系膜上静脉CT值均呈显著线性正相关(P<0.01),而与肠系膜上动脉CT值之间均不具有相关性(P>0.05)。结论正常小肠MSCT动态增强扫描以门静脉期强化最显著,并与肠系膜上静脉强化呈线性正相关,越接近小肠近段其血供越丰富,MSCT动态增强定量测量能为小肠疾病的诊断和病情评估提供有价值的影像学信息。Objective To quantitatively measure contrast enhancement of the normal small bowel with multi slice spiral CT（MSCT）,provide reference standards for diagnosis of the small bowel diseases.Methods Forty three healthy volunteers were selected and underwent abdominal MSCT plain scan and dynamic enhanced scan in arterial,portal venous and equilibrium phases.All images were used to measure the CT values of the duodenums,jejunums,ileum,superior mesenteric arteries and veins.Results The small bowel CT values among different segments were no significant difference（P〈0.05） in plain scan.The most prominent enhancement was found in portal venous phase for all segments of the small bowel（P〈0.01）,and the CT values among different segments of the small bowel were also significantly different（P〈0.01） in each phase of dynamic enhanced scan.The CT values in each segment of the small bowel had a significant positive linear correlation with the CT values of the superior mesenteric vein in each phase of dynamic enhanced scan（P〈0.01）,but had no correlation with the CT values of the superior mesenteric artery（P〈0.05）.Conclusion The most prominent enhancement of small bowel is in portal venous phase during MSCT dynamic enhanced scan and the closer its proximal segment the more rich blood supply.MSCT dynamic enhanced scan and quantitative measurement can provide valuable imaging information for diagnosis and illness evaluation of the small bowel diseases.
