摘 要:目的:采用同步脑电图联合功能磁共振(EEG-fMRI)分析Rolandic癫痫(RE)患者脑区激活区域偏侧性与脑电图(EEG)偏侧性的关系。方法:根据病例选择标准,共纳入19例RE患儿。采用EEG-fMRI技术采集患儿脑功能数据,排除无发作间期痫样放电(IED)的和头动过大的病例,获得12例患儿的fMRI数据。采用广义线性模型(GLM)方法分析IED相关血氧水平依赖(BOLD)信号改变,计算脑区激活偏侧系数及EEG偏侧系数,并分析两者的相关性。结果:左侧及右侧IED为主的组分析显示BOLD信号激活主要位于同侧外侧裂-中央区,个体分析显示10例患儿外侧裂-中央区范围内BOLD信号显著激活。中央后回和中央沟盖区、中央前回脑区激活偏侧系数与EEG偏侧性显著相关(P<0.01)。结论:fMRI上激活脑区和EEG信号偏侧性具有相关性,中央后回和中央沟盖区最适合作为描述RE偏侧性的目标脑区。Objective:The purpose of this study was to compare the features of combined simultaneous electroencephalogram and functional MRI(EEG-fMRI)in different laterality of Rolandic epilepsy(RE)and to analyze the relationship of Rolandic epilepsy laterality features between EEG-fMRI and EEG.Methods:Followed to the selection criteria,nineteen children with RE were recorded,who underwent EEG-fMRI and seven without interictal epileptiform discharge(IED)and with an unacceptable head motion during exams were excluded.Totally twelve children were going through complete fMRI data.Generalized linear models(GLM)was used to analyze the alteration of IED related blood oxygenation level dependent(BOLD)signal changes.Lateralization coefficient(LC)of activated brain regions and LC of EEG were calculated and the correlation of them was analyzed.Results:Group analysis of IED major in left and right hemisphere showed BOLD signal activation in ipsilateral perisylvian central region.Individual analysis displayed BOLD signal activation in perisylvian central region in 10 cases.LC of precentralgyrus showed significant correlation with that of EEG(r=0.715,P〈0.01).Region which consist of postcentralgyrus and rolandic operculum revealed the same result with a modest increase(r=0.745,P〈0.01).Conclusion:Laterality of activated brain on fMRI is correlated with that of EEG signal.The regions of postcentralgyrus combined rolandic operculum and precentralgyrus are optimal area to describe the laterality of RE.
