摘 要:目的分析院内感染标准化创建工作对社区医务人员手卫生的影响。方法收集2012年2—7月226次及2012年8月—2013年1月233次具有洗手指征的场景作为研究对象,分析院内感染标准化创建工作实施前后医务人员的手卫生情况。计数资料采用χ2检验,P<0.05为差异有统计学意义。结果干预前手卫生执行率、合格率分别为57.0%、51.3%,干预后分别为82.8%、84.1%,两组比较差异均有统计学意义(χ2=37.441、56.671,均P<0.05)。干预后医务人员的手卫生执行情况在直接接触患者前、从污染部位移到清洁部位、摘手套后、无菌操作前、接触患者周围环境及物品后、处理药物或配餐前(84.3%、73.7%、85.0%、85.3%、82.6%、81.0%)均较干预前(52.1%、37.5%、30.4%、62.5%、52.4%、47.1%)有显著提高(均P<0.05)。结论标准化创建工作的实施,有助于医务人员手卫生知识的掌握及手卫生的执行,从而有利于降低院内感染率。Objective To analyze the influence of standardization of nosocomial infection on hand hygiene of medical staff in community hospital. Methods Collecting the scenes in which medical staff need to wash hands as research objects(226from February to July 2012 and 233 from August 2012 to January 2013),then analyzing the inspection results of hand hygiene before and after standardization of nosocomial infection. Measurement datas were analyzed by chi square test. P 〈0.05 was considered statistically significant. Results The rates of implementation and correct hand-washing were 57. 0%and 51. 3% respectively before standardization of nosocomial infection,82. 8% and 84. 1% respectively after standardization of nosocomial infection,which were both significantly different( χ2 = 37. 441,56. 671,all P 〈0. 05). There were significant improvement in many aspects( before touching patients directly,moving from contaminated area to clean area,after removing gloves,before aseptic operation,after touching goods besides patients,before dispensing and compound food) after standardization of nosocomial infection(84. 3% 、73. 7% 、85. 0% 、85. 3% 、82. 6% 、81. 0%) than before( 52. 1% 、37.5% 、30. 4% 、62. 5% 、52. 4% 、47. 1%)( all P 〈0. 05). Conclusions Standardization of nosocomial infection was helpful to mastery of hand hygiene,which can decrease the occur of nosocomial infection.
