摘 要:目的探讨细胞块免疫细胞化学在胸腹水细胞学诊断中对提高肿瘤细胞阳性率、鉴别细胞良恶性、确定肿瘤细胞来源的临床意义。方法回顾性分析收集的65例胸腹水涂片、细胞块和免疫细胞化学诊断,并结合相关临床病史。结果 65例胸腹水中有3例涂片中仅见几个肿瘤细胞,而细胞块为阴性,其余62例标本涂片与细胞块检查结果完全相符;其中13例良性病变标本细胞涂片及细胞块均未查到肿瘤细胞或可疑细胞,报告阴性;49例可疑阳性标本或需鉴定恶性肿瘤细胞来源的,通过免疫细胞化学均得到准确结果。结论细胞块免疫细胞化学在胸腹水肿瘤细胞学诊断中对细胞良恶性的鉴别和鉴别组织来源具有良好的应用价值,值得推广,但对提高阳性率方面细胞块不及普通涂片,二者相互对照观察有助于提高判断结果准确性,对于细胞块及免疫细胞化学的应用应注意与临床资料及细胞学形态相结合,做到合理选择,合理应用。Objective To investigate clinical significance of cell mass immunocytochemistry in improving positive rate of tumor cell, identifying benign and malignant cells, determining tumor cell source in hydrothorax and ascite cytological diagnosis. Methods A retrospective analysis along with related clinical data was made on hydrothorax and ascite smear, cell mass, and immunocytochemical diagnosis in 65 cases. Results Among 65 hydrothorax and ascite cases, 3 cases of them had very few tumor cells with negative cell mass. The other 62 cases had totally corresponding smear and detection results. There were 13 cases without tumor cell or suspected cell in benign smear and cell mass, with negative report. There were 49 cases had accurate results for suspected positive smear and malignant cell source by immunocytochemistry. Conclusion Cell mass immunocytochemistry provides good application value for identification of benign and malignant cell and tissue source, and it is worthy of promotion. Its positive rate is lower than normal smear in improvement, and their combination can improve accuracy of diagnosis. Combined clinical data and cellular morphology are necessary for cell mass and immunocytochemistry. It is necessary for rational choice and application.
