床旁超声引导右锁骨下静脉置管在急诊科的应用

摘　　要：目的观察急诊休克病人在床旁超声引导下行右锁骨下静脉穿刺置管的应用。方法 72例急诊休克患者中,采用床旁彩超引导下的右锁骨下静脉穿刺38例(观察组)和传统体表定位组34例(对比组)。比较两分组的穿刺成功率、穿刺次数、穿刺时间及穿刺并发症。结果观察组的首次成功率92.1%,优于对比组61.7%(P<0.05);观察组穿刺次数、穿刺时间均少于对比组(P<0.01);两分组穿刺相关并发症差异无统计学意义(P>0.05)。结论急诊休克病人在床旁彩超引导下行右锁骨下静脉穿刺置管成功率提高明显,缩短穿刺时间,减少穿刺次数。Objective To discuss the clinical significance of ultrasound-guided right clavicle vein puncture catheter in the emergency patients. Methods Seventy-two cases with shock who received right clavicle vein puncture catheter were divided into two group, ultrasound-guided clavicle vein puncture catheter group（observation group, n =38） and clavicle vein puncture catheter group without ultrasound guidance（control group, n=34）. The success rate of puncture, puncture time and puncture complications were observed and compared between two groups. Results The first success rate was92.1% in the observation group and 61.7% in control group. The time of puncture, performance time in observation group were lower than that in control group（P〈0.05）. There were no significant differences in complications between the two groups. Conclusion Doppler-guided right subclavian vein catheterization is a effective way for emergency patients for fast setting venous access.
