摘  要:[目的]通过电镜观察研究弹性内固定重建椎间稳定的方法对软骨终板变化的作用机制,为椎间盘退变的临床治疗提供新的思路和方法。[方法]选取48只6个月龄日本大耳白兔,随机分组,分为实验失稳对照组和实验弹性内固定组,两组均为24只兔,将所有实验用兔进行手术椎间失稳;仅实验弹性内固定组在手术失稳2个月后用椎弓根螺钉和张力钢丝弹性内固定的方法进行L6、7的稳定性重建,均分别在内固定术后2、4、6个月取材。对椎间盘软骨终板用透射电镜观察,以判断软骨终板的退变过程。[结果]弹性内固定能明显阻止椎间盘软骨终板胶原纤维的退变,并可见软骨终板胶原纤维结构有部分恢复。[结论]弹性内固定能有效地阻止椎间盘软骨终板的退变;能有效促进软骨终板的修复。[Objective] To investigate the reconstruction effect of dynamic internal fixation on degenerative cartilage endplate by using electron microscope,so as to provide a new method for the clinical treatment of intervertebral disc degeneration.[Methods] Fifty- four white Japanese macrotia rabbits( provided by the animal experiment center of Neimenggu Agricultural University) weighed within the range of( 2. 5 ± 0. 15) kg,were divided into 2 groups randomly,with 24 rabbits in the destabilization control group,and another 24 rabbits in the experimental dynamic internal fixation group. All rabbits received destabilization operation,while rabbits in the experiment group were given reconstruction surgery with screw- wire dynamic internal fixation at L6、72 months postoperatively. Rabbits were sacrificed at 2,4,6 months after surgery and electron microscope was used to observe the degeneration process of cartilage endplate. The effects were recorded and compared between two groups. [Results] In the experiment group,the dynamic internal fixation effectively decreased the degeneration process of the collagen fibers of the cartilage endplate,and recovery of part of the collagen fibers was observed. While in the control group,no improvement of the degeneration was found. [Conclusion] The dynamic internal fixation can effectively slower the degeneration of the cartilage endplate and partly recover from the degeneration.
