摘  要:目的评价去细胞真皮基质对兔巨大肩袖损伤的修复效果。方法将42只成年雄性新西兰大白兔随机分为3组:巨大肩袖损伤常规手术组(对照组,n=18),去细胞真皮基质治疗组(实验组,n=18),正常组(正常组,n=6),对照组和实验组于术后4、8、12周分批随机取出各组12侧肩袖标本行生物力学测试;正常组于12周取出12侧肩袖标本行生物力学测试。结果生物力学测试显示:在术后各个时间点,实验组腱骨界面的断裂载荷显著高于对照组,差异有统计学意义(P<0.01);但术后12周二者腱骨界面的断裂载荷仍显著低于正常组,差异有统计学意义(P<0.05)。结论去细胞真皮基质修补巨大肩袖损伤可增强其腱骨界面的抗拉载荷,有利于巨大肩袖损伤的愈合。Objective To evaluate the repairing effects of acellular dermal matrix on the massive rotator cuff tear of rabbits. Methods Forty two adult male New Zealand white rabbits were divided randomly into 3 groups: the conventional surgery group of the massive rotator cuff tear(control group, n =18), the group treated with acellular dermal matrix(experimental group, n =18) and the normal group (normal group, n =6). Twelve side rotator cuff samples were selected randomly in batches from each group after 4, 8 and 12 weeks after operation to perform the biomechanical test. Results The biomechanical test showed that the breaking load of the tendon-bone interface in the experimental group was significantly higher than that in the control group at all time p6ints after operation and the difference was statistically significant(P 〈0.01). But the breaking load of the tendon-bone interface at 12 week was still lower than normal group (P 〈0.05). Conclusion In terms of repairing the massive rotator cuff tear of rabbits, aeellular dermal matrix can enhance the tensile load of the tendon-bone interface, thus benefiting the healing of the massive rotator cuff tear.
