摘 要:目的 评价血管内皮生长因子(VEGF)、骨形态发生蛋白(BMP)复合纳米人工骨材料在经皮椎体成形术(PVP)中的应用价值及安全性。方法 将我院2012-01-2013-12间椎体压缩性骨折病例96例按入院时间分组。前48例纳入对照组,常规行PVP手术,术中注入纳米磷酸钙人工骨;后48例纳入研究组,常规PVP手术中注入VEGF、BMP和纳米磷酸钙人工骨复合材料,比例25 mg∶25 mg∶1000 mg。结果 两组术前疼痛评分、功能障碍评分比较差异无统计学意义(P〉0.05);治疗后研究组评分显著低于对照组,说明研究组患者疼痛减轻和功能恢复更好,差异具有统计学意义(P〈0.01)。两组患者在人工骨注入前、注入过程中及结束手术各时间点的血压和SPO2水平比较,差异无统计学意义(P〉0.05)。研究组患者术后椎体前缘复位率与后凸矫正率略高于对照组,但两组间数据比较差异无统计学意义(P〉0.05)。结论 VEGF、BMP复合纳米磷酸钙人工骨应用于PVP,治疗椎体压缩性骨折,可提高疗效和预后,且具有安全性,其作用机制与VEGF和BMP促血管生长和骨形成密切相关。Objective To evaluate the application value and safety of vascular endothelial growth factor( VEGF),bone morphogenetic protein( BMP) composite nano artificial materials for percutaneous vertebroplasty( PVP) surgery treatment. Methods 96 cases of vertebral compression fractures from January,2012 to December,2013 were selected for the study according to the time of admission. The first48 cases were included in the control group,and conventional PVP routine surgery intraoperative were injected nano- calcium phosphate cement. The later 48 cases were included in the study group. Conventional PVP routine surgery intraoperative were injected VEGF,BMP and nano- calcium phosphate cement composites. The proportion was 25 mg: 25 mg: 1000 mg. Results The difference of preoperative pain scores and dysfunction score was not statistically significant( P〉0. 05); after treatment,scores of the study group were significantly lower than the control group,indicating that the study group patients relieve pain and restore function was better,and difference was statistically significant( P〈0. 01). There was no significant difference between BP and SPO2 levels before the artificial injection,injection time points during the end of surgery of two groups( P〉0. 05). Reset rate and vertebral kyphosis correction rate postoperative of the patients in the study group was slightly higher than the control group,but data difference between the two groups was not statistically significant( P〉0. 05). Conclusion VEGF,BMP composite nano calcium phosphate cement applied to the treatment of vertebral compression fractures PVP can improve the efficacy and prognosis,and has security. Its mechanism of VEGF and BMP angiogenic growth and bone formation are closely related.
