闭环靶控镇静对颅脑损伤患者的脑保护作用

摘　　要：目的探讨闭环靶控镇静对颅脑损伤患者脑保护作用的疗效、经济性及安全性.方法随机选取40名颅脑损伤患者,分成对照组和实验组,比较2组患者镇静前、后6、12、24、48 h患者脑组织内葡萄糖(Glu)、乳酸(Lac)、甘油(Gly)的水平,同时监测患者平均动脉压(MAP)、心率(HR);48 h异丙酚总用量;统计患者发生镇静过深、过浅次数变化情况.结果 2组患者葡萄糖水平随治疗时间延长而降低(P<0.05),乳酸和甘油水平随治疗时间延长而升高(P<0.05),实验组葡萄糖、乳酸及甘油含量和对照组差异无统计学意义(P>0.05),实验组平均动脉压(MAP)和心率(HR)均较对照组平稳(P<0.05),实验组48 h异丙酚总用量少于对照组(P<0.05);实验组出现镇静过深、过浅次数均低于对照组(P<0.05).结论闭环靶控镇静对颅脑损伤患者脑保护作用短期疗效显著,且有较好的经济性和安全性.Objective To explore the curative effect, economy and safety of closed-loop target controlled infusion on the cerebral protection for eraniocerebral injury patients. Methods 40 craniocerebral injury patients were randomly divided into control group and experimental group. The changes in cerebral glucose （Glu） , lactic acid （Lac） , glycerol （Gly） , mean arterial pressure （MAP） and heart rate （HR） of patients before calm and after 6, 12, 24, 48 hours were compared between the two groups. 48 hours total amount of propofol and sedation times of too deep and too shallow were calculated and analyzed. Results The level of glucose decreased with increasing time of therapy （P 〈 0.05） , but levels of lactic acid and glycerol increased with treatment time （P 〈 0.05） in the two groups. There were no significant differences in the levels of Glu, Lac and Gly between the two groups （P 〉 0.05） . Compared with the control group, MAP and HR were more stable, 48 hours total amount of propofol was less, and sedation times of too deep and too shallow were lower in the experimental group （P 〈 0.05） Conclusion The closed-loop target controlled infusion showed significant curative effect on the cerebral protection for craniocerebral injury patients, and it also had good economy and security
