单操作孔电视胸腔镜和开胸手术治疗慢性脓胸的疗效比较

摘　　要：目的比较单操作孔电视胸腔镜(Uniport VATS)和开胸手术治疗慢性脓胸的疗效。方法 34例慢性脓胸患者分别应用Uniport VATS及开胸手术行脓胸病灶清除、纤维板剥离术,比较两组的手术时间及术后胸腔引流量、置管时间、住院时间、疼痛程度差别。结果两种方法均无手术并发症及死亡病例。与开胸手术对比,Uniport VATS组的手术时间缩短,术后胸腔引流量、置管时间、住院时间减少,疼痛减轻(P<0.05)。结论 Uniport VATS是治疗慢性局限性脓胸安全、可靠的方法。Objective To compare the clinical efficacy of uniportal video-assisted thoracic surgery（VATS） and thoracotomy for chronic empyema. Methods Thirty-four patients with chronic empyema underwent uniport VATS or thoracotomy for empyema debridement and pleural decortications. The operation time and postoperative volume and duration of chest drainage, hospital stay, and pain intensity were compared between both groups. Results No serious complications and deaths occurred in both groups. Compared with the thoracotomy group, the operation time and postoperative volume and duration of chest drainage,hospital stay, and pain intensity were increased in the uniport VATS group（P〈0.05）. Conclusion The uniport VATS is safe and reliable for chronic localized empyema.
