不同血液透析频次患者的心血管事件风险评估

摘　　要：目的评估不同血液透析(HD)频次患者的心血管事件的风险。方法回顾性分析156例在开平市中心医院HD患者的临床资料,记录其基线资料、合并症、实验室指标等,根据不同透析频次分为每周2次HD(2HD组)及每周3次(3HD组),观察其心血管事件,采用Logistic回归分析评估不同组血透患者的心血管事件的危险因素。结果研究期间共发生心血管事件58(37.2%)例次,其中2HD组28(30.4%)例次,3HD组30(46.9%)例次,3HD组心血管事件发生率明显高于2HD组(P=0.023)。年龄、糖尿病、心血管病史、HD次数、尿量及白蛋白是HD患者发生心血管事件的危险因素;校正糖尿病、心血管病史、白蛋白后,年龄(每增加10岁、每周3次HD及透析前尿量是HD患者发生心血管事件的独立危险因素。心血管病史及透析前尿量是2HD组发生心血管事件的独立危险因素;年龄(每增加10岁)、糖尿病及透析前尿量(尿量每减少100 m L/24 h)是其发生心血管事件的独立危险因素。结论每周2次HD患者的心血管事件发生率较每周3次HD者低,每周2次HD可作为终末期肾病患者较好替代治疗方案之一。Objetive To evelaute on cardiovascular events in patients with dfferent Hemodialysis frequency. Methods A retrospective included 156 cases of HD patients in Kaiping city hospital, Record baseline data, comorbidities, laboratory indicators. Depending on the frequency of dialysis HD group is divided into two times a week group（1） and three times a week group（2）, The main event for cardiovascular events were observed. For Logistic regression analysis we use Logistic Regression Analysis to evaluate different hemodialysis patients risk factors for cardiovascular events. Results During the study period, there were 58 cardiovascular events, including group（1） 28 times, 30 times group（2）, The incidence of cardiovascular events in group（2） was significantly higher than group（1）（P=0.023）, related to age, sex, comorbidities, and laboratory parameters, Group（2） HD risk of cardiovascular events was 2.13 times higher thanj the groups（1）（95% CI 1.78~4.62, P=0.015）. ConclusionHD three times a week may be of higher risk in patients with cardiovascular events in those two times a week. But the results need to be reconfirmed with a multi-center, prospective, large sample study.
