甲氨蝶呤妊娠囊内注射联合清宫术治疗子宫瘢痕处妊娠的临床效果分析

摘　　要：目的评价甲氨蝶呤妊娠囊内注射联合清宫术治疗子宫瘢痕处妊娠的临床效果。方法收集2010年12月~2013年12月本院收治的子宫瘢痕处妊娠患者20例作为观察组,并选取同时住院的20例患者作为对照组,观察组施以甲氨蝶呤妊娠囊内注射联合清官术进行治疗,对照组囊内注射甲氨蝶呤。所有患者每隔3d测定血β-HCG,观察并记录血β-HCG降到正常值的时间、住院时间、治疗费用并做统计分析。结果观察组患者血β-HCG降到正常值的时间、住院时间、住院费用分别为（11.20±1.94）d、（20.15±2.68）d、（4814.20±28.10）元,远低于对照组（30.03±3.24）d、（33.82±4.35）d、（6312.27±38.13）元,两组比较,差异有统计学意义（P〈0.05）。观察组治疗的有效率为100%明显高于对照组90%,两组比较,差异也有统计学意义（P〈0.05）。结论甲氨蝶呤妊娠囊内注射联合清宫术治疗子宫瘢痕处妊娠,具有血β-HCG降到正常值的时间短、住院时间短、治疗费用低,治疗效果好等优点,具有较大的临床推广价值。OBJECTIVE To evaluate the effects of methotrexate combined with the curettage treating the cae- sarean scar pregnancy. METHODS 20 cases of caesarean scar pregnancy which are treated in our hospital from December 2010 to December 2013 were selected to made the observation group. Treatments of gestational sac injec- tion of methotrexate combined with surgery were used in the observation group. While the gestational sac injection of methotrexate was used in the control group（20 cases）. The blood 13-HCG level was monitored every 3 days. The time of [3-HCG dropping to the normal and being in hospital, cost of treatment were observed. RESULTS The time of 13- HCG dropping to the normal and being in hospital of the observation group was （ 11.20 ± 1.94） d and （ 20. 15 ± 2. 68）d,the cost of treatment was （4814.20 ±28.10）yuan,which are lower than the control group （30. 03 ±3.24） d, （33.82 ±4. 35）d and （6312.27 ± 38.13 ）yuan. Effective rate of the observation group was 100% and the control group was 90%. The differences were statistically significant（P 〈 0. 05）. CONCLUSION Applying the gestational sac injection of methotrexate combined with the curettage to treat the caesarean scar pregnancy has advantages of shorter time of β-HCG dropping to the normal, shorter hospital stays and lower cost of treatment. It is worthy of clini- cal application.
