院内跨专科胰岛素泵治疗糖尿病患者的专人规范化管理

摘　　要：目的探讨院内跨专科胰岛素泵治疗糖尿病患者的专人规范化管理。方法 2013年6月至2014年5月期间第三军医大学新桥医院跨专科使用胰岛素泵治疗的糖尿病患者138例(设为实验组),实行专人规范化管理;2012年6月至2013年5月期间未实行专人规范化管理的院内跨专科使用胰岛素泵糖尿病患者133例设为对照组。比较两组患者胰岛素泵治疗过程中血糖的有效控制及各类安全问题的发生情况。结果两组患者的血糖控制效果比较,实验组优于对照组,其中血糖达标时间、血糖水平,高血糖、低血糖、黎明现象发生率比较,差异有统计学意义(均P<0.01);两组设备故障、输注障碍、导管脱落、局部皮肤硬结、导管连接处漏液及注射部位感染的发生率比较,差异有统计学意义(均P<0.05)。结论院内跨糖尿病专科使用胰岛素泵治疗,能使患者血糖在短期内得到有效控制;而实行专人规范化管理,可使血糖控制效果更为理想,并显著地减少安全性事件的发生。Objective To discuss the specialized standardized management of insulin infusion pump in the treatment of diabetes cross departments in the hospital.Methods Totally 138 patients which received insulin infusion pump treatment from June 2013 to May 2014 were selected as experimental group,and received special person management.Other 133 patients from June 2012 to May 2013 ,which had not received special person management, were selected as control group.The effective control of blood glucose and incidences of safety issues were compared between two groups.Results The effective control of blood glucose in experi- mental group was better than which in control group.The differences of time of the blood glucose meet the criteria, level of the blood glucose, and incidences of hyperglycemia, hypoglycemia, dawn phenomenon between two groups had statistical significance （all P〈0.01）. The occurrence of equipment failure, infusion obstruction, catheter loss, local callosity, leakage and injection site infection in two groups were statistically significant （all P〈0.05）.Conclusion The application of insulin infusion pump in non-diabetes department can control the levels of blood glucose effectively in a short time. Levels of the blood glucose were controlled more ideal and safer with the standardized management.
