依托咪酯复合瑞芬太尼与丙泊酚复合瑞芬太尼用于无痛取卵术的麻醉效果对比

摘　　要：目的探讨经阴道无痛取卵中分别采用依托咪酯复合瑞芬太尼与丙泊酚复合瑞芬太尼的麻醉效果差异。方法 61例拟行经阴道取卵患者采用随机数字表法分为依托咪酯组30例,丙泊酚组31例,分别监护并记录二组患者在麻醉诱导前(T1)、麻醉诱导后2min(T2)、手术结束时(T3)时刻的平均动脉压(MAP)、心率(HR)、血氧饱和度(SpO2)值变化情况,并比较二组患者的瑞芬太尼总剂量、麻醉起效时间、意识恢复时间差异;比较二组患者的麻醉效果、术后60min的视觉模拟疼痛评分(VAS)及不良反应发生率差异。结果在T2、T3时刻,依托咪酯组的MAP、HR检测值显著的高于丙泊酚组患者且差异具有统计学意义(P<0.05);二组患者在各个时刻的SpO2检测值比较差异无统计学意义(P>0.05)。依托咪酯组的麻醉起效时间(60.6±10.2)s显著低于丙泊酚组(69.4±11.4)s,且具有统计学意义(P<0.05);依托咪酯组的意识恢复时间(6.5±2.4)min显著高于丙泊酚组(5.2±1.8)min,且具有统计学意义(P<0.05);二组患者的手术时间、取卵个数比较差异无统计学意义(P>0.05)。依托咪酯组麻醉效果优为70%高于丙泊酚组的45.16%,依托咪酯组的麻醉效果分布显著优于丙泊酚组(P<0.05);依托咪酯组的VAS评分(2.36±1.54)分显著低于丙泊酚组的(3.27±1.58)分,且具有统计学意义(P<0.05)。结论经阴道无痛取卵中分别采用依托咪酯复合瑞芬太尼较丙泊酚复合瑞芬太尼具有更好的麻醉效果,手术中MAP、HR波动性更小,术后疼痛程度更轻,但术后意识恢复时间略长。Objective To study of the anesthetic effect of transvaginal painless take by Etomidate combined with Remifentanil and Propofol with Remifentanil were used in eggs.Methods Totally 61 patients underwent transvaginal oocyte retrieval were randomly divided into 31 cases of Propofol group and Etomidate group of 31 cases,respectively,and then monitored and recorded the changes of two groups of patients before induction of anesthesia（T1）,2min after anesthesia induction（T2）,at the end of operation（T3）and mean arterial blood pressure（MAP）,time heart rate（HR）,oxygen saturation（SPO2）,and compared the differences of two groups of patients with Remifentanil total dose,the onset time of anesthesia,the recovery time of consciousness,anesthesia effect,postoperative 60 min pain visual analogue score of the patients in the two groups（VAS）and adverse reaction incidence rate.Results In T2,T3 moment,HR detection MAP,Etomidate group values was significantly higher than the patients in the Propofol group and the difference was statistically significant（P〈0.05）;SPO2in every moment of the two groups were detected value difference was not statistically significant（P〉0.05）.The onset time of anesthesia of Etomidate group was 60.6＋10.2（s）was significantly lower than that of Propofol group69.4＋11.4（s）and had statistical significance（P〈0.05）;Etomidate group consciousness recovery time was 6.5＋2.4（min）,significantly higher than that of Propofol group 5.2＋1.8（min）and had statistical significance（P〈0.05）;Operation time,the two groups of patients with oocyte numbers compared the difference was not statistically significant（P〉0.05）.Etomidate group 70% were higher than that of the anesthesia effect of Propofol group 45.16%,Etomidate group was significantly better than the anesthetic effect of distribution of Propofol group（P〈0.05）;Etomidate group VAS score 2.36＋1.54 was significantly lower than that of Propofol group 3.27＋1.58 points and had statistical signi
