术前呼吸指导对老年肺癌患者围术期心理应激反应及术后肺功能的影响

摘　　要：目的探讨术前呼吸指导对老年(年龄≥60岁)肺癌患者围术期心理应激反应及术后肺功能的影响。方法 2012年10月—2014年10月选取该院收治的128例肺癌手术患者为研究对象,根据随机数字表将患者分为观察组及对照组各64例,对照组围术期行常规性护理,观察组术前接受呼吸指导,对比分析二组围术期心理应激反应及术后肺功能变化情况。结果观察组干预后收缩压、舒张压、心率水平显著低于对照组(P<0.05)。观察组干预后汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分显著低于对照组(P<0.05)。观察组患者术后第一秒呼吸气体的容积(FEV1)、用力呼吸的肺活量(FVC)及FVC占预计值百分比(FVC%)改善优于对照组(P<0.05)。结论术前呼吸指导能有效改善老年肺癌患者围术期心理应激反应,改善患者焦虑、抑郁情绪,提高患者肺功能,促使患者术后康复。Objective To evaluate the impact of preoperative respiratory guidance on psychological stress response and postoperative pulmonary function in patients with elderly lung cancer.Methods Totally 128 cases of lung cancer patient were divided into observation group（n=64）and control group（n=64）from October 2012 to October 2014.The control group was underwented with routine of perioperative nursing and the observation group was underwented with preoperative respiratory guidance.The perioperative psychological stress response and postoperative pulmonary function changes of two groups were compared.Results The systolic and diastolic blood pressure,heart rate of observation group were lower than the control group（P〈0.05）.The Hamilton Anxiety Scale（HAMA）,Hamilton Depression Rating Scale（HAMD）score of observation group were lower than the control group（P〈0.05）.The levels of children with respiratory gas 1s（FEV1）,forced expiratory vital capacity（FVC）and percent predicted FVC（FVC%）of observed groups were lower than the control group（P〈0.05）.Conclusion For the elderly patients with lung cancer,Preoperative respiratory guidance can improve perioperative psychological stress response,improve patient anxiety and depression,improve lung function,promote the rehabilitation of patients after surgery.
