咽鼓管康复在手术治疗无效分泌性中耳炎患儿中的应用价值

摘　　要：目的:探讨咽鼓管康复训练在腺样体切除术治疗无效分泌性中耳炎(SOM)患儿中的应用价值。方法:选择曾采取腺样体切除术而未使SOM症状缓解的70例患儿为研究对象,按照随机分组的方法分为观察组和对照组,每组各35例,观察组给予咽鼓管康复训练,对照组给予鼓膜置管手术,观察两组患儿治疗3个月后临床治疗效果。结果:治疗后3个月观察组和对照组患儿A型比例明显增加,C型比例明显下降,两组与治疗前比较和两组治疗后比较差异均有显著的统计学意义(P<0.01)。观察组患儿治愈率为40.00%,而对照组37.14%,差异无明显统计学意义(P>0.05);治疗后3个月两组纯音听阈气导值均明显下降,差异有显著的统计学意义,(35.21±8.56)vs(20.12±9.78),(34.63±8.55)vs(22.67±10.75),P<0.01;但是两组治疗后差异无统计学意义,(20.12±9.78)vs(22.67±10.75),P>0.05。结论:咽鼓管康复对手术治疗无效分泌性中耳炎患儿具有明显的治疗效果,能提高患儿的听力,值得临床推广应用。Objective：To evaluate the clinical efficacy of eustachian tube rehabilitation training on secretory otitis media after failed operation.Methods：Seventy children with secretory otitis media after failed operation were enrolled in this study and were randomly divided into observation group and control group with 35 in each group.The patients in observation group were treated by eustachian tube rehabilitation training,and those in the control group were treated by myringotomy with grommet insertion.The clinical efficacy was evaluated after 3months.Results：The patients of type A increased and those of type C reduced in both of the groups after 3 months.The significant differences were observed before and after treatment in both groups（P〈0.01）.However,no difference was found after treatment between the two groups（P〉0.05）.The recovery rates in the observation group and control group were40.00% and 37.14% respectively,which was not significantly different（P〉0.05）.The tone hearing threshold gas conductivity was decreased after 3 months in both groups（P〈0.05）,（35.21±8.56）vs（20.12±9.78）;（34.63±8.55）vs（22.67±10.75）,but no difference existed between the two groups（P〉0.05）.Conclusion：The rehabilitation training of eustachian tube can treat secretory otitis media effectively after failed operation,which can be applied widely in clinic.
