综合治疗AECOPD并呼吸衰竭和肺性脑病的临床疗效观察

摘　　要：目的探讨纤维支气管镜联用无创通气、药物治疗慢性阻塞性肺疾病急性加重期并呼吸衰竭、肺性脑病的临床疗效。方法 68例呼吸内科行纤维支气管镜联用无创通气、药物治疗的慢性阻塞性肺疾病急性加重期并呼吸衰竭、肺性脑病患者,观察治疗前及治疗2 h、4 h、12 h后的血气分析变化及临床疗效。结果本组68例患者中55例(80.88%)临床症状、血气分析结果明显改善,与治疗前比较:p H值升高(P<0.05),动脉血二氧化碳分压水平降低(P<0.05),动脉血氧分压水平升高(P<0.05)。结论纤维支气管镜联用无创通气、药物治疗慢性阻塞性肺疾病急性加重期并呼吸衰竭、肺性脑病患者疗效确切,能迅速缓解患者症状。Objective To investigate the clinical efficacy of bronchoscopy combined with noninvasive ventilation,medication treat AECOPD complicated with respiratory failure,pulmonary encep Halopathy.Method The treatment of bronchoscopy combined with noninvasive ventilation,medication was taken to 68 cases of AECOPD with respiratory failure,pulmonary encep Halopathy.Observing the blood gas analysis changes and clinical changes when take the treatment before and 2 h、4 h、24 h after.Results Among the 68 patients treated with this method,the clinical symptom and blood gas analysis results of 55 cases（80.88%）improved significantly.Compared with no treatment：p H values increased（P〈0.05）,arterial partial pressure of carbon dioxide levels decreased（P〈0.05）,arterial oxygen levels increased（P〈0.05）.Conclusion The treatment of bronchoscopy combined with noninvasive ventilation,medication is an effective method for AECOPD complicated with respiratory failure,pulmonary encep Halopathy,remitting clinical symptoms rapidly.
