摘  要:目的探讨农村地区中老年人群的腰围身高比值(waist to height ratio,WHtR)与血压、血脂的关系。方法采用统一编制调查表记录2013年在4个县共4281人的血压、血脂、WHtR等指标。WHtR对血压、血脂具体指标影响采用主成分回归分析方法。结果农村地区人群WHtR≥0.5组高血压患病率比WHtR〈0.5组高20.13%(95%CI:17.33-22.38),WHtR≥0.5组血脂异常患病率比WHtR〈0.5组高16.89%(95%CI:14.20-19.58),WHtR≥0.5组高胆固醇血症、高低密度脂蛋白胆固醇血症、低高密度脂蛋白胆固醇血症、高甘油三酯血症患病率均比WHtR〈0.5组高;主成分回归分析表明,WHtR与其他危险因素相比对收缩压(systolic blood pressure,SBP),舒张压(diastolic blood pressure,DBP),总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C),低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)均有较大影响,标准回归系数bi分别为0.24、0.14、0.06、0.08、0.07、-0.10。WHtR≥0.5与WHtR〈0.5相比SBP的值大0.24个单位,即WHtR≥0.5与WHtR〈0.5相比DBP、TC、TG、LDL-C值分别大0.14、0.06、0.08、0.07个单位,WHtR≥0.5与WHtR〈0.5相比HDL-C的值小0.10个单位。结论农村地区人群WHtR与血压、血脂各指标关系密切,可以通过降低WHtR改善血压、血脂各项指标。Objective To evaluate the association of waist to height ratio(WHtR) with blood pressure(BP) and dyslipidemia in rural population in China.Methods Questionnaires were used to record the BP,serum lipid and WHtR of4 281 rural people in China,2013.And the effect of the WHtR to the BP and serum lipid was analyzed by the method of principal components regression.Results The prevalence of hypertension and dyslipidemia was significantly higher in rural population with WHtR≥0.5 than the normal controls(both P〈0.05).The prevalence of hypertension difference between WHtR≥0.5 and the WHtR 〈0.5 was 20.13%(95% CI: 17.33-22.38) The prevalence of dyslipidemia difference between WHtR≥0.5 and the WHtR〈 0.5 was16.89%(95% CI: 14.20-19.58),the prevalence of hypercholesterolemia,high blood LDL-C,low blood HDL-C and hypertriglyceridemia was high in the group of WHtR≥0.5.Principal components regressions results showed that WHtR had influence on the SBP,DBP,TC,TG,LDL-C,HDL-C,the standardized coefficients of WHtR was 0.24,0.14,0.06,0.08,0.07,-0.10 respectively.WHtR≥0.5 compared to the WHtR〈 0.5,the DBP,TC,TG,LDLC was higher,while the HDL-C is lower.Conclusions Results showed that WHtR is closely related to BP and serum lipid in rural population in China.It might be a feasible way to reduce BP,and serum lipid by controlling WHtR.
