摘 要:目的通过观察2型糖尿病(T2DM)合并骨质疏松患者血浆25羟维生素D[25(OH)D]、白细胞介素-6(IL-6)、胰岛素样生长因子(IGF-1)、血脂及胰岛素抵抗(IR)水平,探讨其相互之间的关系。方法将研究对象分为T2DM组120例、T2DM合并骨质疏松症(OS)组(T2DM+OS组)110例,糖耐量正常者(NGT组)115例,空腹抽血测定25(OH)D、IL-6、胰岛素样生长因子(IGF-1)、糖化血红蛋白(HbA1c)、胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDLC),并计算稳态模型胰岛素抵抗指数(HOMA-IR)进行比较分析。结果 T2DM组25(OH)D、IGF-1、HDL-C低于NGT组,而IL-6、HbA1c,TC,TG,LDL-C、HOMA-IR均高于NGT组,差异有统计学意义(P<0.05);T2DM+OS组25(OH)D、IGF-1、HDL-C低于NGT组,而IL-6、HbA1c,TC,TG,LDL-C、HOMA-IR均高于NGT组,差异有统计学意义(P<0.05);T2DM+OS组25(OH)D、IGF-1低于T2DM组,IL-6均高于T2DM组,差异有统计学意义(P<0.05),而HDL-C、HbA1c,TC,TG,LDL-C、HOMA-IR比较,差异无统计学意义(P>0.05)。结论血浆25(OH)D、IGF-1水平降低及IL-6水平升高可能是T2DM合并骨质疏松的高危因素之一。Objective To observe the level of 25-hydroxy vitamin D[25(OH)D],Interleukin,6 (IL-6),Insulin-Like Growth Factor-l(IGF-1) ,blood lipid and insulin resistance(IR) in patients with type 2 diabetes mellitus(T2MD) and Osteoporosis. To in- vestigate the relationship of 25(OH)D3 with blood lipid and IR. Methods 120 patients with newly diagnosed T2DM group,ll0 pa- tients with type Z diabetes mellitus complicating Osteoporosis(T2DM + os group), 115 individuals with normal glucose tolerance (NGT group, as control) were enrolled. Fasting 25 (OH) I), IL-6, IGF-1, HbA1 c, total cholesterol(TC), triglyeeride(TG), low densi- ty lipoprotein(LDL-C),high density lipoprotein cholesterol(HDL-C) were determined. To compute HOMA-IR and to compare a- nalysis. Results The levels of 25(OH)D,IGF-1, HDL-C were more declined in T2DM group than those in NGT group (P〈0.05). The levels of IL-6, HbAlc,TC, TG, LDL-C, HOMA-IR were significantly higher in T2DM group than those in NGT group(P〈 0.05). The levels of 25(OH)D,IGF-1, HDL-C were more decl^ned in T2DM +os group than those in NGT group (P〈0.05). The levels of IL-6, HbAlc, TC,TG,LDL-C, HOMA-IR were significantly higher in T2DM -}- os group than those in NGT group(P〈 0.05). The levels of 25(OH)D,IGF'l were more declined in T2DM q-OS group than those in T2DM group (P〈0.05). The levels of IL-6 were significantly higher in T2DM -kOS group than those in T2DM group(P〈0.05);the levels of HDL-C, HbAlc, TC, TG,LDL:C,HOMA-IR have no difference comparing T2DM +OS group and T2DM group(P〉0.05), Conclusion The low levels of 25(OH)D,IGF-1 and high levels of IL-6 is a high risk of type 2 diabetes metlitus complicating osteoporosis.
