骨折延迟愈合患者IGF-1、PDGF、ALP、PINP、β-CTX水平变化的研究  被引量:35

摘  要:目的探讨骨折延迟愈合患者血小板衍生生长因子(PDGF)、人胰岛素生长因子1(IGF-1)、碱性磷酸酶(ALP)、Ⅰ型胶原氨基端前肽(PINP)和Ⅰ型胶原羧基端肽β特殊序列(β-CTX)水平的变化及意义。方法选取2012年1月至2014年9月该院骨折延迟愈合患者62例为研究对象(观察组)。另选择同期被该院收治的骨折正常愈合患者58例(对照组)。统计受试者骨折后1、6、12周血清PDGF、IGF-1、ALP、PINP、β-CTX水平。结果骨折1周时,观察组与对照组血清IGF-1、PDGF、ALP、PINP、β-CTX水平比较,差异无统计学意义(P>0.05);但骨折6、12周时,观察组血清IGF-1、PDGF、ALP、PINP、β-CTX水平均低于同期对照组,差异有统计学意义(P<0.01);骨折1、6、12周时,闭合性骨折和开放性骨折患者血清IGF-1、PDGF、ALP、PINP、β-CTX水平差异均无统计学意义(P>0.05);骨折1周时,<60岁与大于或等于60岁患者血清IGF-1、PDGF、ALP、PINP、β-CTX水平差异无统计学意义(P>0.05),但骨折6、12周时,≥60岁患者血清IGF-1、PDGF、ALP、PINP、β-CTX水平均低于同期小于60岁患者,差异有统计学意义(P<0.05);血清IGF-1、PDGF、ALP、PINP、β-CTX与年龄呈负相关(P<0.05),与骨折类型、骨折分型、体质量指数(BMI)等因素无相关性(P>0.05)。结论血清IGF-1、PDGF、ALP、PINP、β-CTX等指标与骨折延迟愈合密切相关,上述指标在骨折延迟愈合患者血清中低表达,是反映骨折延迟愈合较好的指标。Objective To study changes of IGF-1 ,PDGF,ALP,PINP,β-CTX levels of patients withdelayed fracture healing. Methods From January 2012 to September 2014,62 patients with delayed fracture healing were selected as the object of study (ob- servation group);other 58 cases of normal fracture healing patients treated in our hospital at the same period were selected as con- trol group. The subjects' serum PDGF, IGF-1, ALP,PINP, β-CTX contents were recorded at 1 week, 6 weeks and 12 weeks after fracture. Results The serum IGF-1,PDGF,ALP,PINP,β-CTX levels of patients in both group at one week after fracture had no statistical difference (P 〉 0.05), but the serum IGF-1, PDGF, ALP, PINP, β-CTX level of the observation group were lower than that of the control group at six and 12 weeks after fracture (P〈0, 01). The serum IGF-1, PDGF,ALP,PINP,β-CTX level of closed fractures and open fractures patients had no statistical difference at 1 week, 6 weeks, 12 weeks after fracture (P〉0.05) ; the serum IGF-1,PDGF, ALP,PINP,β-CTX levels of 〈60 years old group and ≥60 years old group at 1 week after fracture had no statistical difference (P 〉 0.05)but serum IGF-1,60 PDGF,ALP,PINP,β-CTX levels of ≥60 years old group were lower than 〈60 year- old group at 6 weeks and 12 weeks after fracture (P〈0.05),the serum IGF-1, PDGF, ALP, PINP,13-CTX levels were negatively correlated with age(P〈0.05),and had no obvious correlation with the fracture type, fracture classification,BMI and other factors (P〉0.05). Conclusion Serum IGF-1 ,PDGF, ALP, PINP, ^-CTX levels and other indicators are closely associated with delayed u- nion of fracture,and the low expression of the above indexes in patients with delayed healing of fracture are good indicators for de- layed healing of fracture.
