外伤性OVCF行PVP手术时机与疗效相关性研究

摘　　要：目的观察外伤性骨质疏松性椎体压缩骨折(Osteoporosis Vertebral Compression Fracture,OVCF)行椎体成形术(Percutaneous Vertebroplasty,PVP)手术时机与疗效关系。方法回顾性分析126例患者,将发病至手术时间分为A组(≤1周),B组(1-2周),C组(2-3周),D组(≥3周),比较各组术前、术后48 h VAS、ODI评分差异,记录术后各组止痛措施,术后随访1月,比较各组VAS、ODI评分差异。结果四组术前VAS、ODI评分差异无统计学意义(P>0.05),四组术后VAS、ODI评分较术前差异均有统计学意义(P<0.05),四组VAS、ODI评分均有改善,但术后48 h四组间VAS、ODI评分差异有统计学意义(P<0.05),A组改善最佳,B、C组次之,D组相对最差,四组术后1月VAS、ODI评分差异无统计学意义。结论外伤性OVCF行PVP手术治疗疗效确切,早期手术效果更好。Objective To observe the relationship between operative opportunity and thera- peutic effect of percutaneous vertebroplasty （PVP） in treating traumatic osteoporosis vertebral compression fracture. Methods 126 patients were retrospectively divided into four groups by the time from invasion to operation：group A （≤1 week）, group B （1-2 weeks）,group C （2-3 weeks）,group D（ 93 weeks）. The differences of VAS and ODI scores preoperation and 48 hours after operation were compared, and recorded the measure to alleviate pain post-operation,after one month follow-up, compared the difference of VAS and ODI scores among them. Results There were no significant differences of VAS and ODI scores preoperation（P〉0.05）,but had sig- nificant differences of VAS and ODI scores between pre and post-operation （P〈O.05）,while there had significant differences of VAS and ODI scores among them 48 hours after operation（P〈O.05 ）. They were all improved,group A was the best,then group B and C, group D was the worst rel- atively,but there were no significant differences of VAS and ODI scores among them after one month later. Conclusion PVP has obvious therapeutic effect in treating traumatic osteoporosis vertebral compression fracture,and the therapeutic effect will be better if operating early,the patients with relatively bad effect will change better through adjunctive therapy.
