摘  要:目的探讨经阴道超声子宫内膜指标预测早期输卵管妊娠的价值。方法应用相关子宫内膜超声诊断指标对121例临床疑为早期输卵管妊娠的病例进行前瞻性研究。所有病例均经手术病理或临床长期随访确诊,其中早期输卵管妊娠51例,早期宫内妊娠70例。将相关子宫内膜超声指标的预测结果与临床最终诊断结果对照,评价各指标的诊断价值。结果 121例患者内膜厚度≤8 mm共44例,内膜厚度>8 mm共77例;内膜Ⅰ型共35例,Ⅱ型共26例,Ⅲ型共60例;内膜对称共55例,不对称共66例。将内膜厚度≤8 mm、Ⅰ型内膜及内膜对称作为诊断早期输卵管妊娠的阳性指标,其敏感性分别为70.59%、64.71%及88.24%,特异性分别为88.57%、97.14%及85.71%。结论仅根据子宫内膜厚度预测早期输卵管妊娠价值有限,结合内膜形态分型和内膜对称性来预测早期输卵管妊娠有较高的诊断价值。Objective To investigate the value of endometrial index in predicting early tubal pregnancy(ETP) by transvaginal ultrasonography.Methods One hundred and twenty-one cases of clinically suspected ETP were prospectively studied by related endometrial index.51 cases of ETP and 70 cases of early intrauterine pregnancy were confirmed by surgical pathology or long-term follow-up.Value of endometrial index with transvaginal ultrasonography was evaluated by comparing the results of prediction with finally clinical diagnosis.Results In 121 patients,there were 44 cases with endometrial thickness≤8 mm,77 cases with endometrial thickness 〉8 mm.The cases of endometrium in Ⅰ-type,Ⅱ-type,Ⅲ-type were 35,26,60,respectively.Endometrium was symmetric in 55 cases,asymmetry in 66 cases.Endometrial thickness ≤8 mm,Ⅰ-type endometrium,symmetric endometrium could be served as the diagnostic criteria of ETP,and their sensitivity and specificity were70.59%,64.71%,88.24%,and 88.57%,97.14 %,85.71%,respectively.Conclusion Combination of endometrial thickness,echo type,and symmetry can play higher value in predicting ETP because merely endometrial thickness has some limitations in diagnosing ETP.
