鼻窦炎患者钩突变异的影像分析

摘　　要：目的：探讨鼻窦炎及非鼻窦炎患者钩突变异的差异情况。方法随机选取2013年2月至8月126例鼻窦炎及130例非鼻窦炎患者的冠状位CT资料，比较鼻窦炎组及非鼻窦炎组钩突在其气化、内移、外移和肥大等的变异情况。结果鼻窦炎组气化、内移、外移，肥大的发生率分别为3.2％、12.8％、7.9％、8.7％，非鼻窦炎组气化、内移、外移，肥大的发生率分别为2.3％、5.4％、6.2％、3.1％，钩突内移及肥大的发生率在鼻窦炎组与非鼻窦炎组差异显著（P〈0.05），而气化、外移组间比较无差异，总体变异发生率鼻窦炎组（33.3％）高于非鼻窦炎组（16.9％）。结论鼻窦炎组钩突总体内移变异、肥大的发生率高于非鼻窦炎组，鼻窦炎患者钩突总体变异为33.3％，较大一部分患者钩突为正常，正常钩突有重要的生理功能，内窥镜鼻窦手术中允许如条件允许应尽量保留部分正常钩突。Objective To explore the the variations of uncinate process between people with and without sinusitis from CT image.Method 125 patients with sinusitis and 130 without sinusitis were chosen from February to August 2013 in third affi liated hospital of WenZhou Medical University. Compare the anatomy variations of uncinate process such as ： pneumatisation of the uncinate process, inwardly and outwardly curved uncinate process ,and large uncinate process.Result The incidence of pneumatisation, inwardly and outwardly curved uncinate process and large uncinate process in the sinusitis group are 3.2%、12.8%、7.9%、8.7% .In the non- sinusitis group the variations are2.3%,5.4%,6.2%,and3.1%correspondingly. There are remarkable difference from the respect of inwardly curved uncinate process and large uncinate process. The overall incidence of variations in the sinusitis group is higher than that of non- sinusitis group（33.3% versus 16.9%, with signifi cant difference）.Conclusion The inwardly curved uncinate process and large uncinate process in patients with sinusitis have the higher incidence than people without sinusitis.The The overall incidence of variations in the sinusitis group is 33.3%,that is, the uncinate process in many patients with sinusitis are normal. Since normal uncinate process have many important physiology functions, the preservation of normal uncinate process in endonasal endoscopic surgery is recommended.
