RTX配合中药治疗男性慢性盆底疼痛综合征的临床研究

摘　　要：目的 观察辣椒辣素类似物（Resiniferatoxin,RTX）配合中药治疗男性慢性盆底疼痛综合征（CPPS）的疗效.方法 按入选和剔除标准选择48例门诊就诊的CPPS患者,随机分三组,每组16例.A组单纯行经尿道膀胱灌注RTX,B组行经尿道膀胱灌注RTX＋口服中药方剂,C组单纯口服中药方剂.前列腺液常规均提示白细胞阴性,灌注RTX一次,服用中药方剂2周;观察治疗前后盆底疼痛、尿频、尿急症状（PUF评分）、尿道闭合压、生活质量评估（QOL）、症状复发情况并比较各组疗效.结果 A组总有效率为50.00％,B组93.75％,C组56.25％;3个月后复发A组6例,B组2例,C组5例,组间比较差异有统计学意义（P＜0.05）;B组治疗后PUF评分、尿道闭合压、QOL明显优于A组及C组,组间差异有统计学意义（P＜0.05）;不良反应（急性胃肠炎、过敏、尿道灼热感等）方面三组比较差异无统计学意义（P＞0.05）.结论 应用RTX配合中药方剂治疗男性慢性盆底疼痛综合征患者有良好疗效,对提高生活质量、达到内外兼治、减少复发有积极意义.Objective To observe the effect of resiniferatoxin （RTX） combined with traditional Chinese medicine （TCM） in the treatment of male chronic pelvic pain syndrome （CPPS）.Methods 48 cases of male CPPS were randomly divided into three groups,16 cases in each group.Group A was treated with intravesical RTX perfusion,group B was treated with intravesical RTX perfusion combined with TCM,group C was treated with TCM.EPS of all cases had no WBC,intravesical RTX perfusion was done once,the course of TCM treatment was two weeks.Evaluated pelvic pain syndrome and urinary syndrome,urethral closure pressure,quality of life （QOL） and recurrence of three groups.Results The total effective rate of group A,B,C was 50.0％,93.75％,56.25％ respectively.There were 6,2,5 cases of recurrence in group A,B,C respectively after 3 months,there were statistically significant differences among three groups （P＜0.05）.PUF score,urethral closure pressure,QOL in group B after treatment were significantly better than those in group A,C （P＜0.05）.There were no statistically significant differences in the rates of acute gastroenteritis,anaphylaxis,chaude-pisse among three groups （P＞0.05）.Conclusions RTX combined with TCM has good effect in the treatment of male CPPS patients,and has active significance to improve quality of life,help internal and external treatment,and reduce recurrence.
