中西医结合治疗晚期非小细胞肺癌54例

摘　　要：目的:观察中西医结合治疗晚期非小细胞肺癌(NSCLC)的近期疗效及对生存质量、细胞免疫功能的影响。方法:将105例晚期NSCLC患者随机分为治疗组54例和对照组51例。对照组采用奥沙利铂(L-OHP)联合培美曲塞(PEM)四周期化疗方案,治疗组在对照组的基础上口服益气活血汤,化疗结束后1个月,比较两组临床疗效、KPS评分、药物毒性反应及T细胞亚群变化。结果:治疗组化疗后KPS评分为(76.3±4.9)分显著高于对照组的(73.2±4.5)分(P<0.05);治疗组有效率为44.4%,对照组有效率为29.4%,两组有效率比较,差异有统计学意义(P<0.05)。治疗组化疗后CD4+(41.3±6.2)%、CD4+/CD8+(1.55±0.35)数值显著高于对照组[(34.8±5.8)%、(1.36±0.31)(P<0.05)。结论:中西医结合治疗晚期NSCLC一定程度上增强T细胞介导的细胞免疫,提高抗肿瘤效果,改善患者生存质量。Objective： To observe the short-time clinical curative effect of therapy of integrated traditional Chinese and Western medicine on late non-small-cell lung cancer（ NSCLC） and the influence on survival quality and cell immunity function. Methods：One hundred and five patients with late NSCLC were randomly divided into the treatment group and the control group,with 54 cases and 51 cases respectively. The ones in the control group were given chemotherapy program with oxaliplatin in combination with pemetrexed. While the ones in the treatment group were given Decoction of Nourishing Qi and Activating Blood（ DNQAB） for oral administration on the basic treatment of the control group. One month later after chemotherapy was over,the clinical curative rates,KPS scores,toxicity reaction and T-cell subsets changes of both groups were compared. Results： After chemotherapy,KPS score of the treatment group was（ 76. 3 ± 4. 9）,which was much higher than that of the control group（ 73. 2 ± 4. 5）（ P〈0. 05）.The effective rate of the treatment group was 44. 4%,while that of the control group was 29. 4%; And the difference between them had statistical meaning（ P〈0. 05）. After chemotherapy,CD4＋（ 41. 3 ± 6. 2） %,and CD4＋/ CD8＋（ 1. 55 ± 0. 35） of the treatment group were much higher than those of the control group：（ 34. 8 ± 5. 8） % and（ 1. 36 ± 0. 31） respectively（ P〈0. 05）. Conclusion：Therapy of integrated traditional Chinese and Western medicine can strengthen mediated T-cell immunity on late NSCLC to some degree,improve effect against tumor and patients＇ survival quality.
