纤维内镜检查在基层医院吞咽障碍评估中的应用研究

摘　　要：目的探讨纤维内镜检查在基层医院吞咽障碍评估中的临床应用价值。方法选取本院2013年5月-2015年4月收治的86例吞咽障碍患者作为研究对象,分别采用X线造影和纤维内镜两种方法进行检查,以吞咽时X线造影检查结果为金标准,分析纤维内镜检查吞咽障碍的结果,统计纤维内镜检查的灵敏度和特异度,采用Kappa检验评价两者对吞咽障碍严重程度分级检查结果的一致性。结果纤维内镜检查吞咽障碍各级别的灵敏度均在90%以上,特异度均高达100.00%。经Kappa检验,两种检查方法的一致性较强,Kappa值为0.94。结论纤维内镜检查具有较高的灵敏度和特异度,是一种可靠的吞咽障碍评估方法,有利于指导患者的饮食管理和康复治疗,值得临床推广应用。Objective To explore the clinical application value of fibre endoscopy for the assessment of pharyngeal swallowing disorder in the primary hospital. Methods 86 patients with pharyngeal swallowing disorder in our hospital from May 2013 to April 2015 were selected as the research object.Two methods of X-ray and fiber endoscopy were used to examine the pharyngeal swallowing disorder.The examination result of X-ray was selected as the gold standard. The result of fibre endoscopy for the assessment of pharyngeal swallowing disorder was analyzed.The sensitivity and specificity for fibre endoscopy was statisticed.The consistency of the Kappa test were used to evaluate their swallowing pharyngeal disorder grading examination result. Results The sensitivity of different levels of endoscopic examination in swallowing disorder was more than 90% and the specificity was 100%.By Kappa test,the consistency of two methods was strong,the Kappa value was 0.94. Conclusion Fiber endoscopy has higher sensitivity and specificity.It is a reliable annexation of pharyngeal disorder assessment method to guide patient diet management and rehabilitation therapy,it is worthy of clinical promotion and application.
