七氟烷联合i-gel喉罩通气在小儿短小手术麻醉中的应用效果

摘　　要：目的探讨七氟烷联合i-gel喉罩通气在小儿短小手术麻醉中的应用效果。方法选取2014年1-11月本院收治的80例行小儿斜疝术和包皮切除术患儿作为研究对象,随机分为观察组与对照组,各40例。观察组采用七氟烷i-gel喉罩通气全身麻醉,对照组采用氯胺酮静脉注射全身麻醉。比较两组的临床效果、并发症发生率、FLACC评分、苏醒时间。结果观察组T1时的Sp O2水平显著低于对照组,差异有统计学意义（P〈0.05）。观察组T1、T2、T3、T4时的HR、MAP水平显著低于对照组,差异有统计学意义（P〈0.05）。对照组T1、T2、T3、T4时的HR、MAP水平显著高于T0时,差异有统计学意义（P〈0.05）。观察组的并发症发生率为20.0%,显著低于对照组的67.5%,差异有统计学意义（P〈0.05）。观察组的FLACC评分显著优于对照组,差异有统计学意义（P〈0.05）。观察组的苏醒时间为（10.14±3.55）min,显著短于对照组的（20.04±4.68）min,差异有统计学意义（P〈0.05）。结论七氟烷联合i-gel喉罩通气在小儿短小手术麻醉中的应用效果显著,可有效降低并发症发生率,缩短苏醒时间。Objective To explore the application effect of sevoflurane combined with i-gel laryngeal mask airway in anesthesia of pediatric short-time surgery. Methods 80 children patients received pediatric surgery of indirect hernia and circumcision in our hospital from January 2014 to November were selected and randomly divided into the observation group and the control group,40 cases in each group.The observation group was given general anesthesia with sevoflurance combined with i-gel laryngeal mask airway,the control group was given general anesthesia with intra- venous injection of ketamine.The clinical effect,the incidence of complication,the score of FLACC and awakening time in two groups was compared. Results The level of SpO2 at T1 in the observation group was lower than that in the control group,with significant difference（P〈0.05）.The level of HR and MAP at T1,T2,T3 and T4 in the observation group was lower than that in the control group,with significant difference （P〈0.O5）.Thc level of HR and MAP at T1,T2,T3 and T4 in the control group was higher than that at T0,with significant difference （P〈0.05）.The incidence rate of complication in the observation group was 20.0%,which was lower than that 67.5% in the control group,with significant difference （P〈0.05）. The score of FLACC in the observation group was better than that in the control group,with significant difference （P〈 0.05）.The awakening time in the observation group was （10.14±3.55） rain,which was shorter than that （20,04±4.68） min in the control group,with significant difference（P〈0.05）. Conclusion The application effect of sevoflurane combined with i-gel laryngeal mask airway in anesthesia of pediatric short-time surgery is significant,which can effectively reduce the incidence rate of complication,shorten the awakening time.
