摘 要:目的采用循证医学Meta分析方法比较自动、半自动和手动方法测量肝体积的准确性。方法在Cochrane图书馆、Medline、Embase、中国生物医学文摘数据库和全文数据库(Springer、OVID、万方数据库等)检索CT或MRI测量全肝体积的中英文文献,根据纳入文献中所记录的手动、半自动或自动方法分组,采用统计分析软件R的Meta软件包对提取信息进行统计分析。结果共纳入16篇文献,36项研究,315例病人。手动法、半自动法和自动法三组影像方法测量肝体积和实际肝体积差异均无统计学意义,其中手动法影像测量和实际肝体积的加权均数差值为12.098 m L(95%CI:-26.392~50.588 m L),P=0.538;半自动法为26.186 m L(95%CI:-29.842~82.213 m L),P=0.360;自动法为57.144 m L(95%CI:-10.770~125.059 m L),P=0.099。各组汇总结果之间差异也无统计学意义(Q=0.15,P=0.930)。结论手动、半自动及自动方法都是准确的肝体积影像测量方法,自动、半自动方法可以大大提高测量效率,具有很好的推广价值。Objective To compare the accuracies of manual, semi-atuomatic and automatic whole liver volumetry with Meta-analysis. Methods Cochrane library, abstract databases(MEDLINE, EMBASE, and China biology medicine disc) and full text databases(Springer, OVID, CNKI, Wan Fang, et al) were searched for published papers on whole liver volumetry with CT or MR modality. Eligible papers were grouped according to the volumetry method(manual, semi-automatic and automatic) and evaluated for quality. With Meta-analysis package Meta of R software, group-wise Meta-analysis and comparison were performed. Results Sixteen articles including 315 patients met the inclusion criteria. There was no heterogeneity among included studies and there were no significant differences between radiologically estimated and true liver volumes of every group. The weighted mean difference was 12.098 m L(95%CI:-26.392~50.588 m L) in the manual group(P=0.538), 26.186 m L(95%CI:-29.842~82.213 m L) in the semi-automatic group(P=0.360) and 57.144 m L(95%CI:-10.770~125.059 m L) in the automatic group(P=0.099). The pooled results did not significantly differ among the three groups(Q=0.15,P =0.930). Conclusion The manual, semi-automatic and automatic liver volumetry are comparable in accuracy while the semi-automatic and automatic volumetry could greatly shorten measuring time, which are worthy of recommendation in clinical setting.
