晚期结直肠癌脾气亏虚证患者差异表达基因的临床研究

摘　　要：目的:从分子生物学水平探讨晚期结直肠癌脾气亏虚证的中医证候本质,为证候的客观化研究提供思路。方法:选取2012年12月—2013年11月本院肿瘤二科住院及门诊收治的经病理明确诊断的III期、IV期结直肠癌患者,其中晚期结直肠癌脾气亏虚证患者5例为试验组,非脾气亏虚证患者6例为对照组。两组患者均采集外周血,提取RNA,采用Agilent表达谱芯片筛选湿热内蕴证的差异表达基因,并进行基因功能注释及富集度分析。结果:试验组与对照组比较共筛选出126条差异表达基因,其中上调的71条,下调的55条;基因功能富集度分析提示:在上调的差异基因功能中,明显的趋势是脾气亏虚证大部分症状与细胞质基因及免疫应答相关基因的过度表达相关,脘腹胀痛,神疲乏力可能与线粒体基因的上调相关;黏附基因表达下调可能是导致是脾气亏虚证患者免疫功能紊乱原因。结论:应用表达谱芯片筛选差异表达基因,可以从分子生物学水平对晚期结直肠癌脾气亏虚证的证候实质的探索提供新思路;脾气亏虚证与细胞质基因及免疫应答相关基因的过度表达相关,与免疫功能紊乱相关。Objective： To explore TCM syndrome essence of syndrome of deficient spleen qi due to late colon and rectum cancer from the aspect of molecular biology level and provide way of thinking of objective syndrome research. Methods： The clinic patients who were treated in the hospital＇s Second Department of Tumor from December of 2012 to November of 2013 and precisely diagnosed as colon and rectum cancer III-stage and IV-stage. Among them,5 cases with syndrome of deficient spleen qi due to late colon and rectum cancer were regarded as the experiment group,and 6 cases with non- syndrome of deficient spleen qi were regarded as the control group. The peripheral blood of the patients in both groups were collected; Their RNA were extracted; The difference expression gene of syndrome of damp-heat interior accumulation was screened by adopting Agilent expression spectrum chip; And gene function notes enrichment degree were analyzed. Results： Comparing the experiment group with the control group,126 expression genes were screened,among of which there were 71 up-regulation,and 55 were down-regulation. In terms of gene function enrichment degree analysis,in up-regulated difference gene functions,the obvious tendency was that most symptoms of syndrome of deficient spleen qi had something to do with cytogene and related immune response genes; Distending pain of gastric cavity and fatigue might have something to do with up-regulation of mitochondrial genes. The down-regulation of adhesive gene expression might be the reason for immune function disorder of patients with syndrome of deficient spleen qi. Conclusion： Using expression spectrum chip to screen difference expression gene may provide a new way of thinking for syndrome essence of syndrome of deficient spleen qi due to late colon and rectum cancer from the aspect of molecular biology level. Syndrome of deficient spleen qi has something to do with the excessive expression of related immune response genes and immune function disorder.
