早期导泻联合肠内营养治疗重症急性胰腺炎临床研究

摘　　要：目的:观察早期导泻联合肠内营养治疗重症急性胰腺炎的临床疗效。方法:将62例患者随机分为对照组30例和观察组32例。对照组给予常规治疗,全程治疗给予全胃肠外营养;观察组在对照组基础上,给予早期导泻联合肠内营养,观察两组患者腹痛和腹胀缓解时间、腹部压痛消失时间、发热消退时间、血淀粉酶恢复正常的时间、并发症发生率、病死率、住院时间及住院费用等方面的差别。结果:观察组与对照组在腹痛和腹胀缓解时间、腹部压痛消失时间、发热消退时间、血淀粉酶恢复正常的时间、并发症发生率、住院时间及住院费用等方面比较,差异有统计学意义(P<0.05)。但两组病死率比较,无统计学意义(P>0.05)。结论:早期导泻联合肠内营养治疗重症急性胰腺炎能够较早地缓解腹部症状,缩短住院时间,减少住院费用,有利于患者的恢复。Objective ：To observe the clinical curative effect of early catharis combined with enteral nutrition in the treatment of severe a- cute pancreatitis. Methods ：62 patients were randomly divided into control group of 30 cases and observation group of 32 cases. Control group was given conventional treatment and the whole treatment was given total parenteral nutrition. The observation group was given early catharsis combined with enteral nutrition. Then observe the difference of the two groups in abdominal distension and pain, abdomi- nal tenderness disappearance time ,fever regression time, blood amylase returned to normal, complications occurred rate, mortality rate, hospital stay and cost of hospitalization etc. Results ： Comparison of observation group and control group in abdominal pain and abdomi- nal distension time, abdominal tenderness disappearance time,fever subsided time, blood amylase returned to normal, complications oc- curred rate, mortality rate, hospitalization time and cost of hospitalization and so on, the difference was statistically significant （P 〈 0. 05）. But comparison of the mortality rate of two groups has no statistical significance （ P 〉 0.05 ）. Conclusion： Early catharsis com- bined with enteral nutrition in the treatment of severe acute pancreatitis can earlier relieve abdominal symptoms, shorten the hospitaliza- tion time ,reduce the cost of hospitalization ,and is conducive to the patient＇s recovery.
