踝关节滑膜炎的关节镜诊疗策略及疗效评价

摘　　要：目的分析踝关节滑膜炎的关节镜诊疗策略并评价疗效。方法 2005年1月-2012年6月开展踝关节镜检查及镜下治疗踝关节滑膜炎患者46例（50踝）,采用改良Mc Guire踝关节评分系统对术前情况、术后疗效及满意程度进行评定。结果该组46例患者均获随访,随访时间3～29个月,平均15.6个月,术前评分（60.94±10.45）分;最后随访评分（81.58±6.98）分,升高（20.64±7.70）分,术后改良Mc Guire评分较术前提高,差异有显著性（P=0.000）。优26例,良18例,可4例,差2例,术后优良率为88%。结论踝关节镜作为一种微创的诊断及治疗方法,具有创伤小、出血少及恢复快的优点,只要了解其诊疗策略,掌握适应证,规避并发症,关节镜将是治疗踝关节滑膜炎的有效治疗手段。【Objective】To explore the arthroscopic treatment strategies for ankle synovitis and evaluate the therapeutic effectiveness.【Methods】46 cases（50 ankles） were operated with arthroscopic from Jan 2005 to Jan 2011.The degree of pain with VAS scores were analyzed and the efficacy and satisfaction were evaluated with the Mc Guire scores.【Results】46 cases were followed for 3 to 29 months（mean 15.6 months）. The postoperative Mc Guire score was（81.58±6.98）, and was（20.64±7.70） higher than the postoperative score（60.94±10.45）. The two figures have significant statistically difference（ P〈0.001）. In all cases, 26 cases were excellent, 18 cases were good, 4 cases were passable and 2 cases were bad. Excellent and good rate was 88%.【Conclusion】As a minimally invasive diagnostic and treatment, arthroscopic has the advantages such as less trauma, less bleeding and faster recovery. Understanding its diagnosis and treatment strategies, mastering the indications and avoiding complications, arthroscopic will be the effective treatment of ankle synovitis.
