摘 要:目的探讨逆向顺流冲洗拦截法经输尿管镜超声碎石清石治疗输尿管结石的可行与安全性。方法观察组30例,在输尿管镜下直视插入将5 F单J管至结石上方,退镜后将单J管置于镜外,进镜至结石部位,单J管接灌注泵行生理盐水冲洗,使用瑞士第五代超声碎石清石系统(EMS-5)将结石击碎,并通过负压吸附系统将结石清除。对照组32例,逆行冲洗气压弹道碎石。结果观察组1例结石移位,清石率96.67%。对照组7例结石移位于肾盂,结石清石率为78.13%。两组均无输尿管损伤及尿脓毒症等并发症发生。结论逆向顺流冲洗拦截法经输尿管镜超声碎石清石治疗输尿管结石的清石率高,安全可行,值得推广应用。【Objective】To discuss the reverse flow rinsing and intercept method feasible and safety therapy of ureteral stones by ureteroscope ultrasonic lithotripsy. 【Methods】The experimental group of 30 cases, in ureteroscopic direct insert 5 F single J tube to the stone above the ureteroscope, the single J pipe was arranged in the ureteroscope, and then put into the stone location, single J tube infusion pump for physiological saline, using the fifth generation Swiss lithoclast master(EMS-5) broken stone, and through negative pressure adsorption system stone was sucked out of the body. A control group of 32 cases, retrograde flushing pneumatic lithotripsy was carried out.【Results】The experimental group of 1 stone shift, stone clearance rate was 96.67%. A control group of 7 cases stones in renal pelvis stones shift, stone clearance rate was 78.13%.The two groups had no injury of the ureter and urinary sepsis occurred. 【Conclusion】 Reverse flow rinsing and intercept method ureteroscopic ultrasound lithotripsy in treatment of ureteral calculi stone clearance rate is, safe and effective, and is worthy of popularization and application.
